Vascular access specialists work directly with patients, but their influence doesn't stop there.

They may help a bedside nurse work through a concern about an existing line. They may discuss device selection with a provider or help a care team understand the implications of different access options.

Those conversations can help others recognize concerns, understand the reasoning behind a recommendation, and make more informed decisions about patient care.

It's one of the things that makes this specialty different. The knowledge developed through individual patient encounters can become a resource for an entire care team.

If you're an experienced RN interested in putting your clinical skills to work in this kind of specialized role, Vascular Wellness is growing.

For a six-year-old who has endured nearly 30 surgeries, another IV start can carry more weight than the procedure itself.

Mina has Crouzon syndrome and a history of difficult intravenous access (DIVA). After repeated failed attempts during previous procedures, she had developed significant anxiety around IV insertion. With another surgery approaching, her family wanted to avoid repeating that experience, so they contacted Vascular Wellness.

When a Vascular Wellness nurse clinician met Mina at home on the morning of surgery, she faced two connected challenges: establishing reliable vascular access and helping a frightened child feel comfortable enough to participate in her own care.

That meant taking time to understand Mina's previous experiences, build trust with her family, and involve Mina in the process.

What happened next mattered not only for Mina, but for the team preparing to care for her at the hospital.

How the clinician approached this difficult access situation, and what the outcome meant for Mina's hospital stay, is the focus of this Nurse Clinicians in Action clinical case.

Placing the line is one part of vascular access. Knowing what needs to happen before, during, and after that procedure is another.

It means understanding the patient's therapy, considering the available options, and recognizing when the situation calls for a different approach.

It also means knowing that a technically successful placement is only part of the picture. The device needs to serve the patient’s clinical needs, not simply provide access.

That broader perspective is what makes vascular access a specialty. The work calls for more than procedural skill. It calls for an understanding of how access decisions fit into the patient's care.

For experienced RNs interested in developing their vascular access expertise, we're growing our team.

A patient's place in their health journey should never determine the level of care and attention they receive.

At Vascular Wellness, Hospice patients receive the same meticulous vascular access assessment we provide every patient... with their individual needs, comfort, history, and circumstances guiding the decision.

In this Nurse Clinicians in Action case, an elderly Hospice patient needed longer-term vascular access to support pain management. When her access became painful and stopped functioning as expected, her clinician didn't simply look for the next available line.

He took the time to assess the patient, consider her history and symptoms, and work with the care team to determine the most appropriate approach for her individual needs.

Because providing vascular access isn't just about placing a line. It's about understanding the patient who needs it.

There are certain things you can say as a [vascular access] nurse that make perfect sense to another nurse… but sound completely unhinged to everyone else.

“That looks angry.”
"We've got a DIVA"
“When did that start?”
“What’s their baseline?”
“I need to get a look at the other side”

You know exactly what we mean... What would you add to the list?

One year ago, we celebrated a significant advancement for nursing in South Carolina: South Carolina Board of Nursing Advisory Opinion #79, confirming that appropriately trained RNs may place ultrasound-guided small-bore internal jugular central venous catheters.

For Vascular Wellness, this milestone represented years of clinical expertise, advocacy, education, and persistence.

Whitney Page was instrumental in the advocacy that helped move this advancement forward.

Curt Tyler had already completed his training and was ready to put those skills to work when this option became available in South Carolina.

And this year, Josh Rennebaum, VA-BC, completed his training, bringing the number of Vascular Wellness clinicians placing these lines in South Carolina to three.

Since the advisory was issued, more facilities have begun supporting this service, and we’ve seen the impact firsthand: helping prevent delays in care, avoiding unnecessary patient transport, and preserving vessels for patients who may need dialysis.

A year ago, we were celebrating what could be possible... Today, we’re seeing it make a difference at the bedside.

We’re grateful to the South Carolina Board of Nursing, our clinical partners, and everyone who helped make this advancement possible... and especially proud of the nurses helping turn it into better options for patients.

Whitney Page, Curt Tyler, Josh Rennebaum
