Understanding heart failure begins with clear, practical information.

Palm Springs Post Acute was pleased to join the Chelmsford Senior Center for a heart failure presentation led by Veronica Gates, Senior Director of Specialized Services. Veronica explained what heart failure is and shared practical guidance for managing it day to day.
