Today we’re celebrating Ranae Weyer, Research Associate Manager at 3Dhealth, on her 7th work anniversary!

We asked Ranae a few questions to mark the occasion:

What are you most proud of contributing at 3Dhealth this past year?
“Over the past year, I have played a pivotal role in our team’s growth by actively participating in the hiring process and leading the end-to-end training for all new Research Associates, ensuring they were successfully integrated and equipped to contribute effectively.”

What’s one way you’ve grown, professionally or personally, since joining 3Dhealth?
“I have significantly built my confidence in training others and advanced my expertise in 3Dhealth projects by successfully collaborating across project teams.”

Curious what keeps Ranae inspired at 3Dhealth? See below. ⬇️

Congratulations, Ranae! Thank you for helping grow and guide our Research team, and for everything you bring to 3Dhealth. Here’s to 7 years and many more!

Most health systems plan their physician workforce in FTEs. So do we. Every Provider Development Plan we build uses the FTE as a measure of clinical time.

But a Primary Care physician at the 25th percentile of MGMA productivity serves about 990 patients. At the 75th, about 1,700. Same specialty, same 1.0 clinical FTE.

The FTE measures time. It doesn't measure what that time produces.

So we measure patient access three ways:

→ Verified Access: is the practice open to new patients, confirmed by calling rather than asking.
→ Access Velocity: how long a new patient waits.
→ Served Lives: how many patients that FTE actually manages.

Five weeks, one at a time, with our own data. We call the gap between clinical time and patient access the FTE Fallacy.

Introducing The Readout: 3Dhealth's quarterly executive briefing. First issue out today.

Three minutes. What we're seeing, what it means, and what healthcare leaders should do next, straight from our own data.

This quarter: The FTE Fallacy. Two physicians can be identical on paper, same specialty, same 1.0 clinical FTE, and still differ by roughly 700 patients in who they actually serve.

We'll be taking that apart here over the next five weeks.

Over the last six weeks, we've shared what the data reveals about rural General Surgery in America.

A pipeline producing a fraction of the surgeons needed. A workforce already past capacity. A retirement wave with no replacement. Access barriers closing the front door. And patients with nowhere else to go.

The numbers are sobering. But they are not a reason for paralysis: they are a reason for honest, data-led planning.

Every community deserves access to the right physicians. Rural communities deserve that too.

The rural General Surgery crisis is not one problem. It is five simultaneous failures.

1. A pipeline producing 13-22 rural surgeons per year against 1,700 open positions.
2. A workforce already operating at or above capacity.
3. A retirement wave removing 1.5 FTEs of access per surgeon lost.
4. Restricted access already hitting 1 in 4 practices and rising.
5. Consumers who won't wait and increasingly have nowhere else to go.

No single policy fixes all five. But understanding all five is where honest planning has to start.

11% of General Surgery practices are effectively unreachable. Voicemail. Extended hold. No path to schedule.

Not because those practices don't exist, but because when a new patient calls, no one answers. In 18% of practices with wait data, the wait is 31 days or longer.

The headline shortage is real. But the access deficit, the gap between a physician who is listed and one who is actually reachable, is where the real story lives.

It's presentation day at the NMHA Annual Meeting.

Today, Ron Flower takes the stage with Nobody Answered: What 1,500 Calls Reveal About Access in New Mexico. 16 years of secret shopper calls, 1,500 of them placed to New Mexico practices, a statewide access picture most executives are seeing for the first time, and a clear view of what a health system can fix without recruiting.

In 2022, 6.7% of General Surgery practices were classified as restricted. By 2024, that number had reached 25.9%, nearly 1 in 4 practices. In less than two years.

3Dhealth's benchmark dataset doesn't measure theoretical capacity. It measures what a new patient actually encounters when they pick up the phone.

In rural markets, where the next option is 60-90 miles away, a restricted practice isn't an inconvenience. It's the end of the road.
