Denials don't take a day off, but every team needs to.

Since we spend our days fighting for providers, this summer we found ways to let off some steam. Trading appeal packets for pickleball paddles, throwing axes, and baseball bats.

Because the fun days make the hard days easier, and the people make it all worth while.

A little Valentine’s Day fun for healthcare.

We made these cards for our team and for everyone working tirelessly across hospitals, health systems, and rev cycle teams. You deserve more than a candy heart, but we hope this is a start.

Happy Valentine’s Day. 💙

Is your ambient scribe actually increasing denial risk?

Ambient scribes have gotten really good at transcription.
That part is largely solved.

The harder problem? Documentation intent.
Accurate transcription ≠ accurate clinical documentation. And auditors evaluate intent, not just words.

This creates an impossible tradeoff:
🔸 Document only what was said → leave gaps auditors flag
🔸 Document everything → bury physicians in noise
Either way, denial risk goes up.

The real concern emerges when ambient scribes move beyond note-taking into RCM optimization. Capturing complexity without understanding baseline conditions or clinical reasoning is a fast way to trigger audits.

Ambient scribes reduce typing. They don't replace clinical thinking.

The question isn't whether your scribe hears the visit. It's whether it understands the patient.

How are you thinking about this tradeoff?

As teams plan priorities for the year ahead, we put together a practical framework to help revenue cycle leaders bring clarity to vendor selection and initiative planning.

The challenge is rarely knowing that problems exist.
It is turning that intuition into shared baselines, clear priorities, and defensible decisions when evaluating new tools.

This video introduces a framework to get started.
The full guide goes deeper, with decision matrices, scoring templates, and evaluation tools you can apply directly to your own environment.

If automation or AI initiatives are on your roadmap this year, the guide to choosing the right option is linked below.

❄️ Day 12 of 12 Days of Denials: Atelectasis

The final denial of the series reminds us that not every finding is a billable diagnosis.

Small areas of atelectasis show up all the time on X-rays, but without symptoms, impact on care, or treatment, payers see it as incidental.

In this video, we highlight why documentation of clinical significance is key, otherwise, even a “real” diagnosis won’t survive review.

From day one to day twelve, we’ve uncovered what makes or breaks a claim. Here’s to ending the year denial-free!

🎄Day 11 of 12 Days of Denials: Type 2 MI

Type 2 MI denials almost always come down to one thing: the chart doesn’t fully connect the troponin rise to true ischemia.

These cases sit in the overlap between stress-induced injury and actual imbalance of oxygen supply and demand. This is a point payers look closely at to see how clearly that distinction is documented.

Today’s case shows why partial details aren’t enough and which elements have to be in the record to move a troponin bump out of “injury” territory.

The countdown is almost over, just one more denial left to close out the series!

🌨️ Day 10 of 12 Days of Denials: COPD

When a patient arrives visibly worse than their baseline and the treatment matches the severity, the documentation should speak for itself.

In today’s case, the chart had everything it needed, yet the payer still pushed back. This is a good reminder that strong details aren’t just helpful, they’re your leverage on appeals.

We’re getting close to the end of the series, and the denials aren’t getting any softer.

🎁 Day 9 of 12 Days of Denials: Congestive Heart Failure

Some denials come down to one simple problem: the chart doesn’t say *what kind* of heart failure the patient actually has.

HF denials are rising fast, and the biggest issue isn’t the testing, it’s the missing details.

Let’s look at why payers push back when the story isn’t specific and how clearer documentation turns a vague diagnosis into a defensible one.

Stay tuned for more denial deep-dives!
