Great patient care doesn't end when a patient leaves the hospital.

That's where people like Terri Foti, RN, Case Manager at Acadia St. Landry Hospital, make a difference.

Case management is an important part of the care we provide, helping patients and families navigate their healthcare journey, coordinate services, and prepare for a safe transition home or to the next level of care.

Terri plays an important role in connecting the pieces, working alongside our healthcare team to ensure patients have the support and resources they need beyond their hospital stay.

In recognition of Case Management Week, we're proud to recognize Terri for her dedication, compassion, and commitment to the patients and families we serve.

Happy Case Management Week, Terri! Thank you for all you do for ASLH and our community!

At Acadia St. Landry Hospital, keeping care close to home is more than a phrase.

For Dr. Jacob Clement, it means helping patients stay near the people who support them. It means building trust over time. It means understanding that every patient brings more into the exam room than a diagnosis.

Those values are part of why we are so proud to celebrate Dr. Clement being named Best Physician in Acadiana’s Choice.

His recognition is an individual honor, but his story is also a reflection of the kind of care we work to provide every day at Acadia St. Landry Hospital.
