Hello!
Here is a quick story of a recent resident we had discharge from our facility.
When a recent resident arrived at our facility, they needed short-term rehabilitation following a hospital stay that had left them feeling weak and uncertain about returning home independently.
Upon admission, our interdisciplinary team worked closely with the resident to create personalized therapy and care goals. Through daily physical and occupational therapy, encouragement from staff, and determination on the resident's part, steady progress was made each week.
What began as needing assistance for simple daily tasks soon transformed into renewed strength, improved mobility, and increased confidence. The resident celebrated milestones along the way, from walking 5 feet to walking distances to regaining independence with everyday activities.
After successfully completing their rehabilitation program, the resident was able to return home safely to family and the comforts of their own environment.
Before discharge, they shared:
"The support, kindness, and therapy I received here helped me regain my strength and confidence. I'm so grateful for everyone who helped me along the way."
We are proud to have been part of this resident's recovery journey and wish them continued health and happiness at home.
Nothing makes us happier than helping our residents reach their goals and return home stronger than before.
If you find yourself in this position or feel like reconditioning and strengthening is needed, feel to reach out on here, or by calling the main phone (707) 994-7738 and ask for Admissions. We, as members of Meadowood's Community, would love to assist you with any challenges in your way!
