Strong partnerships create stronger communities, and we're grateful for partners like Pathways Community Center who are committed to finding new ways to connect individuals with the resources they need.

Through a shared commitment to serving others, our partnership has grown to create additional opportunities for patients to access medication assistance. By welcoming our Community Outreach Advocate onsite in the Learning Lab each month, Pathways has helped make it easier for community members to learn about Welvista's services, receive application assistance, and access the support they need.

Their collaborative approach and willingness to think creatively have strengthened connections between organizations and expanded access to care for the individuals and families they serve. In just a short time, this partnership has already helped connect more patients to life-changing resources, and we're excited about the impact we'll continue to make together.

We're grateful to Pathways Community Center for their partnership and dedication to improving the health and well-being of the communities they serve.
