Can you maintain control when the task gets faster and less predictable? šŸ‘€

For this individual with ataxia, we’re using BlazePods to challenge dynamic seated postural control, reactive balance, coordination, and accuracy while reaching outside of his base of support.

The weighted vest provides added proprioceptive input while increasing the demand on the trunk to maintain stability throughout the task.

Build a stable foundation at the trunk → then challenge everything moving around it. šŸ’Ŗ

Progress isn’t always about one number — it’s about what those numbers allow you to DO. šŸ§ šŸ’Ŗ

Over the past few months, Brian has made significant gains in balance, walking speed, endurance, and overall independence. When he first started, we utilized body-weight support for almost everything in the clinic. Today, we rarely need it.

šŸ“ˆ July → September
• TUG: 55.85 sec → 32.58 sec
• Berg Balance Scale: 22/56 → 38/56
• 6-Minute Walk: 216 ft → 350 ft
• 10-Meter Walk: 22.91 sec → as fast as 11.16 sec
• His daily step count has also continued to increase

And even more importantly, we’re seeing these improvements translate into real functional changes. He can now perform stand-pivot transfers independently, complete a 360° turn without assistance, walk without an assistive device with CGA–Min A, and perform step-ups with Min A without using his upper extremities for support.

His goal? Walk without a walker.

There’s still work to do, but every step with less assistance brings him closer. šŸ‘šŸš¶ā€ā™‚ļøšŸ§

Not just a strength exercise. šŸ§ šŸ’Ŗ

Incorporating Turkish get-ups into upper extremity recovery challenges:
→ Motor planning
→ Sequencing
→ Coordination
→ Scapular stability
→ Core activation

Over here, we love functional neuro intervention and recover.

POV: you play Twister at work šŸ¤øā€ā™€ļøšŸ”“šŸŸ¢šŸ”µ

But there’s a lot more going on here than just hitting colored targets!

For this individual with ataxia, we’re using the parallel bars to reduce degrees of freedom and create a more controlled environment while he practices reaching outside his base of support.

A big focus is learning how to strategically position and reposition his hands for stability while the rest of his body completes the task.

Why does that matter? Because this strategy carries over to real-life mobility — using a walker, grab bar, pole, countertop, or another stable surface to create support while performing transfers and navigating the environment.

Sometimes improving independence isn’t about taking support away — it’s about learning how to use it effectively. šŸ§ šŸ’Ŗ

Minimal foot clearance? Look beyond the ankle. šŸ‘€

Successful swing requires coordinated hip flexion, knee flexion, and ankle dorsiflexion, all happening quickly enough to advance the limb for the next step.

For this individual with hereditary spastic paraplegia, a strong extensor pattern makes accessing that flexion strategy particularly challenging.

So rather than assisting the movement, we increased the demand: ankle weights + the client’s Hypershell, which was set to RESIST rather than assist hip flexion.

After challenging limb swing using resistance, stairs and hurdles for error augmentation— we incorporated speed to challenge how quickly those components can come together during walking.

āš”ļø Transcutaneous spinal stimulation (TSS) was applied to support motor recruitment throughout gait training.

⚔ See it. React to it. Move!

Reactive balance doesn’t always have to mean recovering from a push or a loss of balance. For individuals with Parkinson’s disease, we can also challenge the ability to quickly process an external stimulus and respond with an appropriate movement.

Catching, reaching, and reacting to objects can challenge:

šŸ‘€ Visual processing & tracking
🧠 Reaction time
šŸ¤ Hand-eye coordination
⚔ Movement initiation and speed
ā†”ļø Dynamic weight shifting
šŸŽÆ Anticipatory timing and motor planning

Parkinson’s can make initiating and scaling movement more difficult, so adding an unpredictable target encourages the body to react instead of overthink the movement.

Catch it, reach for it, respond to it — all while maintaining control of your body in space. 🧠⚔

Let’s talk TRANSFERS šŸ‘

The key to a successful transfer? Head–hips relationship.
Head–hips relationship. Head–hips relationship. Do I need to say it again? šŸ˜‚

When we move the head and trunk in one direction, the hips can move in the opposite direction — helping create the momentum and clearance needed for a more efficient transfer.

It’s not always about being stronger. Sometimes it’s about using the right strategy. šŸ§ šŸ’Ŗ

Posture, weight shift, and propulsion—all with one setup. šŸ”„

Posterior band resistance challenges this individual with incomplete SCI to actively shift forward, drive through the hips, and generate propulsion rather than relying on a flexed posture with minimal push-off.

We added activator poles to reinforce a more upright trunk while practicing forward + backward walking and getting those glutes working. šŸ”„

For someone who also struggles with retropulsion during sit-to-stand and stand-to-sit, this gives us repeated practice controlling her center of mass forward against a posterior pull.
