This Meet the Team Monday, we’re highlighting Troy Spires, DNP-FNP, at our Fairfax site!

Troy joined LCHCS in 2024 and provides family medicine care for patients across a wide range of ages, with a special interest in diabetes care.

His dedication to health care extends into the classroom as an adjunct professor at USC Salkehatchie, where he teaches clinical nutrition and nursing courses. Outside of work, Troy enjoys boating and golf!

We’re grateful to have Troy on Team Low Country and for the care he provides to our Fairfax community.

Your health doesn’t stop between visits, and neither does our care!

Low Country Health Care System is now offering Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) to eligible patients. These services help your care team stay connected with you between office visits and monitor important health measurements, such as blood pressure, weight, and blood sugar, from the comfort of your home.

If a change needs attention, your care team can reach out early and provide additional support. It’s another way we’re helping you stay informed, connected, and in control of your health.

Ask your LCHCS provider if RPM or CCM may be right for you.
