SSM Health Adult Day Services recently celebrated 40 years of service with a time capsule burial honoring the countless participants, families, staff members and volunteers who have been part of the journey.

For four decades, the program has helped adults with complex health needs remain safely in their homes and communities longer through a unique blend of medical care, social connection and personalized support.

Touching the lives of so many patients who have walked through our doors, we're grateful for donations that allow us to continue providing excellent care while also investing in the future. Every October, unites to support those fighting breast cancer. This year, we collectively donated $6,000 to the SSM Health Greater Fond du Lac Cancer Center in Fond du Lac. We’re proud to stand alongside an organization that provides compassion and care to local families when they need it most. To every teammate who gave, thank you. Your kindness is what makes Holiday worth the trip.

Making an impact, one putt at a time in our community and supporting women's health services! 🏌️SSM Health Waupun Memorial Hospital will see enhanced care and resources for women in the community thanks to a donation from the Fox League Ladies Golf League.

The Agnesian HealthCare Foundation recently received $4,900 from the proceeds of the annual Sh***un for the Cure golf outing. Your generosity makes a lasting difference for local patients and families.

We are now officially in the Medicare Advantage & Prescription Drug Plan Annual Enrollment Period (AEP) lasting until December 7th.

Those enrolled in a Medicare Advantage plan or a standalone Medicare Prescription Drug plan (PDP) may experience changes to their plan benefits and/or premium for 2027.

We encourage patients to review their Annual Notice of Change (ANOC) letter to understand changes for 2027. Access to basic necessities like clothing and hygiene products can have a meaningful impact on a person's health and well-being.

We're grateful to receive a $3,000 grant from the Green Bay Packers Foundation that will help the SSM Health Samaritan Clinic and Agnesian HealthCare Foundation provide essential items for uninsured and underinsured patients facing financial hardship.

The funding will help purchase clothing and hygiene supplies, allowing our team to address immediate needs identified during clinic visits and provide practical support when patients need it most.

"Community support like this helps ensure the SSM Health Samaritan Clinic can continue caring for some of the most vulnerable members of our community," said Deb Winterhack, Agnesian HealthCare Foundation director of development, Greater Fond du Lac.
