The CMS Long-term Enhanced ACO Design (LEAD) Model introduces a ten-year performance period, the longest of any CMS ACO model to date, along with flexible payment structures and enhanced support for high-need patient populations.

For primary care practices evaluating this opportunity, understanding the details matters. How does LEAD change the economics of shared savings? What does it take to qualify, and how do the two risk options stack up?

Vytalize Health answers these questions and more on our CMS LEAD Model FAQ page, covering:
• Payment structures and how they support upfront investment
• Benchmarking changes and the Administrative Add-On
• Support for high-need and dually eligible patients

Value-based care continues to evolve, and staying informed helps practices make confident, data-driven decisions about participation. Explore the details and see how your organization can prepare: vytalize.me/4k7nAKO

Network design isn’t a back-office function anymore; it’s a strategic lever for value-based care.

Vytalize Health’s Northeast President, Michael Ruiz de Somocurcio, will bring the IPA perspective to two panels at the NAACOS Fall 2026 Conference: “Identifying, Rewarding, and Scaling High-Performance Providers” and “Aligning Around the Patient: Payer and Provider Partnerships in Action.”

Attending the conference? Come join the conversation. Not attending? Stay tuned, we’ll share key takeaways.

It's Primary Care Week, and we want to start with a simple thank you.

To every primary care physician who knows their patients by name, remembers the details, and shows up year after year: you are the foundation of our healthcare system.

Primary care is where health actually happens. It's where chronic conditions get managed before they become crises, where early signs get caught, and where patients have someone in their corner who sees the whole picture. The evidence is consistent: communities with strong primary care live longer, healthier lives at lower cost.

And yet independent primary care practices have been asked to do more with less for decades. Rising administrative burden. Shrinking margins. Pressure to consolidate or close.

That's why Vytalize Health exists.

We believe independent primary care physicians should be rewarded for keeping people healthy, not just for seeing more patients. So we built a value-based care solution that gives them the data, the clinical support team, and the financial model to practice medicine the way they always wanted to.

But supporting practices one at a time isn't enough. The decisions that shape primary care are made in Washington, so that's where we show up. Our team has been on Capitol Hill advocating for the physicians we serve: pushing for payment models that reward keeping patients healthy, a lasting commitment to value-based care in Medicare, and the freedom of independent practices to stay independent. Primary care deserves a seat at that table, and we intend to keep showing up to make sure it has one.

To our partner physicians: thank you for trusting us to be part of your team. Happy Primary Care Week!

Summer is officially over, and so is our Culture Club's Vytalize Summer Outside Challenge.

This summer, we challenged our team to get outside, log hours in nature, and share their adventures. The goal? 2,000 hours together. The result? 46 Vytalizers logged 3,044 hours outside.

From hiking trails in Cyprus to moonrises over New Smyrna Beach, our team showed up and showed out. Being remote-first doesn't mean going it alone; our team proves it every time!

At RPA's Advocacy & Innovation Weekend, Vytalize's Dr. Mufaddal Kheda made a compelling case: CMS's LEAD Model is reshaping how nephrology practices and ACOs can work together.

Under LEAD, nephrology practices can partner with ACOs as Preferred Providers and align under the Non-Primary Care Capitation (NPCC) payment model. This goes well beyond a new payment option. It fundamentally changes the relationship.
Nephrologists no longer need to rely on referrals alone. They now have a path to co-own value-based outcomes for some of the highest-risk, highest-cost patients in any ACO population.

Which practices will earn these partnerships? According to Dr. Kheda, the winners share four traits:
✓ They lead with outcomes data, including optimal dialysis start rates, home dialysis utilization, and transplant referral rates.
✓ They position themselves as cardio-kidney-metabolic (CKM) specialists, not siloed kidney specialists.
✓ They accept value-based payment aligned with accountability.
✓ They build Centers of Excellence for complex, high-risk kidney disease populations.

Together, these traits reflect a single shift: from delivering services to delivering measurable value.

The market is ready. ACOs need nephrology partners who can demonstrate value and accept shared accountability for outcomes. This is the path forward.

Vytalize Health was one of two ACOs representing independent primary care physicians at NAACOS's Fall Advocacy Summit and Hill Day. By taking our advocacy directly to Capitol Hill, we worked to protect and strengthen the accountable care models that physicians and patients depend on.

The results speak for themselves: ACOs have generated $37.4 billion in Medicare savings while improving patient care and outcomes. The case for continued investment in accountable care has never been stronger.

Vytalize Health has submitted formal comments on the proposed 2027 Physician Fee Schedule, advocating for stronger ACO payment, simpler quality reporting, and better-protected shared savings for independent primary care practices.

We applaud CMS for engaging Accountable Care Organizations for input when it comes to a proposal that could meaningfully affect practices' finances and reporting requirements. Read the full breakdown of what we asked for and why it matters: vytalize.me/4jiWakM
