MissionCare was built on years of specialized experience caring for individuals whose medical, behavioral and social needs can make traditional skilled nursing placement extremely difficult.

WFSB recently shared the story of Francis Smith and his final years at 60 West. His experience is a strong example of the type of complex case that helped shape the MissionCare model — and why specialized expertise, the right clinical resources and an individualized approach to care are so important for this population.

Today, MissionCare continues to build on that experience, providing skilled nursing options for individuals who may otherwise have very few appropriate places to turn. 💙

Francis Smith, who spent 70 years in prison while maintaining his innocence, died this summer at 101, listed by the Connecticut Department of Correction as the longest-serving U.S. prisoner.

A recent national story involving a resident of our sister center, 60 West, is shining a light on a challenge that extends far beyond any one individual or facility.

As people with complex medical, behavioral and social needs grow older, hospitals, correctional systems and other public agencies increasingly need appropriate long-term care options for individuals who can be extraordinarily difficult to place.

That is where MissionCare can make a difference.

At MissionCare at Holyoke, our work is rooted in the belief that complex circumstances should not prevent someone from receiving appropriate skilled nursing care in a safe, respectful and stigma-free environment.

It is difficult work at the intersection of healthcare and public policy — and an increasingly important part of meeting the needs of vulnerable populations.

That is the mission behind MissionCare.

Francis Smith was one of a growing number of elderly people who are incarcerated.

What makes a care center truly feel like home?

For David Skoczulek, Vice President of Business Development and Communications at iCare Health Network, it comes down to creating an environment where residents feel comfortable, connected and genuinely at home.

In this clip from the latest episode of Elevate Elder Care, David shares what success in long-term care looks like from his perspective — and why “home-like” has to be more than a catchphrase.

MissionCare Health and iCare Health Network were honored to be represented this month at the National Conference of State Legislatures meeting on parole and prison release.

Dave Skoczulek, Vice President of Business Development and Communications, had the opportunity to speak on the topic of medical and geriatric parole, including the real-world challenges and opportunities surrounding long-term care placement for justice-involved individuals with complex medical needs.

These patients are often among the most clinically complicated and difficult to place in healthcare, requiring thoughtful coordination across long-term care, behavioral health, public safety, corrections, and public policy.

We are grateful for the opportunity to share perspective alongside Sarah Anderson and to learn from legislators, policy leaders, corrections professionals, and healthcare stakeholders from across the country.

These are important conversations, and they serve as a meaningful reminder that healthcare and public policy are deeply connected.

What started as a blank canvas became a beautiful reflection of our community. 🎨

As part of National Skilled Nursing Care Week, residents and staff at MissionCare at Holyoke participated in iCare Health Network’s collaborative art initiative inspired by this year’s theme, “Moments in Bloom.”

Using their own creativity, memories, and ideas, our team transformed a three-foot by three-foot canvas into a personal expression of who we are at MissionCare at Holyoke. Each detail reflects the thought, care, and heart that went into bringing this piece to life.

We are so proud to have contributed to this meaningful exhibit celebrating creativity, connection, and community across iCare Health Network.

MissionCare at Holyoke fired up the grill for a fun and delicious cookout!

Residents and staff enjoyed good food, fresh air, and time together - the perfect recipe for a great day.

During National Skilled Nursing Care Week, our friends at 60 West created a beautiful video that captures the heart of what happens inside MissionCare centers every day. 🌸

“How I Bloom” shares the voices of residents and staff as they reflect on connection, compassion, second chances, meaningful moments, and the relationships that help people feel supported and valued.

While this video was filmed at 60 West, its message speaks to the work we believe in across MissionCare at Holyoke: meeting people where they are, honoring their individuality, and creating moments that help them bloom.
