CNSide Diagnostics https://cnside-dx.com/ CNSide Diagnostics, LLC is a company dedicated to developing and commercializing proprietary clinical diagnostic laboratory assays.

CNSide® is a highly sensitive, cerebrospinal fluid-based, laboratory developed test (LDT) for patients with Leptomeningeal Metastases (LM) from carcinomas and melanomas. The first-in-class CNSide™ CSF assay is performed in a CLIA-registered facility in Houston, Texas at the request of top cancer physicians in the U.S. CNSide™ is a highly sensitive, cerebrospinal fluid-based, laboratory developed test (LDT) for patients with Leptomeningeal Metastases (LM) from carcinomas and melanomas. By enabling quantitative analysis and molecular characterization of tumor cells, we provide powerful insights to healthcare providers that guide patient care and treatment decisions.

CNSide now offers more ways to potentially gain insights from cerebrospinal fluid.

Starting October 12, physicians in the U.S. can order the following CNSide® CSF Molecular Profile tests:

- CNSide® CSF Molecular Profile - Solid Tumor (DNA & RNA)
- CNSide® CSF Molecular Profile - Hematologic Malignancies (DNA & RNA)

Molecular Profile looks at more than 300 cancer-related genes in cell-free DNA and more than 1,600 transcripts by RNA sequencing, enabling broad characterization of genomic and transcriptomic alterations, including sequence variants, copy-number alterations, and gene fusions; in addition, the Hematologic Malignancies test supports molecular assessment of clonality.

The tests also take CNSide beyond leptomeningeal metastases (LM) and can be used for primary CNS tumors, solid tumors that have spread to the CNS, and hematologic cancers involving the CNS.

For patients with suspected or confirmed LM, Molecular Profile can also be ordered alongside CNSide® CSF TCE (Tumor Cell Enumeration) when clinically appropriate.

Diagnosing LM can be challenging when conventional findings are inconclusive.

A retrospective, single-institution analysis that evaluated 12 patients with suspected LM comparing CNSide® CSF TCE and NGS with standard MRI and CSF cytology and using a cutoff of >0 CSF tumor cells/mL, CNSide demonstrated:

- 77.8% sensitivity
- 100% specificity
- 100% positive predictive value
- 88.3% accuracy

CNSide detected CSF tumor cells in 7 of 12 patients, supporting the potential role of CSF tumor cell quantification as a complementary tool to MRI and CSF cytology when evaluating suspected LM.

The study also highlights the potential of integrating CSF tumor cell analysis with NGS to provide additional molecular insights in patients with CNS-involved metastatic disease.

CNSide Diagnostics has signed an in-network provider agreement with Blue Cross Blue Shield of Michigan for the CNSide® CSF Tumor Cell Enumeration assay, effective September 1, 2026. BCBSM serves approximately 5 million commercial and Medicare Advantage members.

CNSide® now holds contracted reimbursement coverage representing approximately 158 million covered lives nationwide.

This achievement is part of our ongoing commitment to:
- Expanding patient access to innovative diagnostics
- Supporting clinicians with actionable diagnostic insights
- Advancing options for the LM community

For your patients with suspected or confirmed leptomeningeal metastases, that means one fewer barrier between a clinical question and a quantitative CSF answer.

For patients with LM, tracking disease over time can be challenging. However, serial CSF monitoring may provide additional insight into disease burden and treatment response.

In a case of a rare, aggressive CNS tumor with leptomeningeal dissemination, serial CNSide® CSF TCE tests provided longitudinal insight across 7 time points, all while CSF cytology and routine CSF parameters remained unremarkable.

This study by Pelletier et al. demonstrates the potential of serial CSF tumor cell monitoring to provide a complementary view of disease burden and treatment response during complex, multimodal therapy.

For physicians managing patients with LM, more data from the CSF may offer another perspective when tracking what’s happening below the surface.

What molecular story is the CNS telling?

This clinical data highlight is from a prospective UCSF study of patients with metastatic breast cancer and leptomeningeal metastases (LM) and shows why looking directly at the CSF matters.

- 50% of patients with confirmed LM had molecular alterations detected by CSF NGS
- 71.4% of tested patients had CSF tumor cells detected by CNSide®

CSF and extracranial NGS showed variable concordance and discordance, highlighting the potential for CNS-specific molecular insights that may not be captured outside the CNS. CNSide identified tumor cells and pathogenic/actionable mutations in patients with negative CSF cytology.

What if we stopped managing leptomeningeal metastases the way we always have?

The future of LM care will require more than traditional diagnostics. It will require better data, deeper molecular insight, and a more precise understanding of what’s happening in the CNS.

At our recent symposium at the SNO/ASCO CNS Metastases Conference, leading experts in neuro-oncology explored what that future could look like from CSF-based diagnostics and molecular profiling to AI, data analytics, and emerging therapies.

The conversation is changing. Is your approach keeping up?
