Nobody told Rachelle Kuramoto that midlife was her cue to slow down. Or if they did, and she ignored them.
A little over a week ago, our co-founder ran the Berlin Marathon and set a new personal record. Through the Brandenburg Gate, across the finish, and straight to a b**r in front of the Reichstag.
What I love most about these photos isn’t the medal. It’s her face. That is a woman having the time of her life.
The story we usually hear about menopause is a story of loss. Slow down, scale back, manage the decline. Some women have real symptoms in this stage that need real care. Women in this stage are also setting PRs, starting companies, and figuring out who they are when nobody else gets to decide.
Both are true. Every woman’s transition looks different. Rachelle’s looks like this.
Swipe to the end to see how she felt about it. 😉

Yesterday I published an essay built on a Trevor Noah joke. The same day, I sat in the audience and watched him speak. I'm still processing that.

Here's the part I love. Trevor never said the line my essay is built on. A friend texted me about a bit of his comparing pregnant women to collectible dolls: priceless in the package, ordinary once opened. That isn't the joke he told. But her version was the truer one.

That is what a great wordsmith does. His words landed hard enough to keep working after the laugh, reshaping themselves in someone else's head until they named something real: how loudly we celebrate women while they're pregnant, and how quiet it gets the moment they deliver.

Watching him on that stage, I kept thinking about how much words matter, and how much more they matter when action follows. Most pregnancy-related deaths happen after delivery. Most are preventable. We are cutting the coverage that prevents them.

Thank you, Trevor Noah, for jokes that keep teaching long after the punchline. And thank you, advertisingweek, for an amazing lineup.

The federal government is proposing a $5,000 baby bonus for new mothers. It just cut the Medicaid coverage that keeps those same mothers alive after they deliver. I've been sitting with this piece for months. I'm done sitting with it.

Same hormone but when through a different route there is a different blood clot risk.

Missed Part 1? Start there. It covers where the clot warning around hormone therapy came from, and why one pill tested in 2002 shaped the conversation for two decades.

Now the new data. Researchers followed more than 1 million women in Denmark and compared estradiol pills, estradiol through the skin, and no hormones at all.

Pills were linked to a small rise in blood clots at every dose, and to more strokes and heart attacks at doses above 1 mg a day taken for more than a year. Patches, gels, and sprays showed no increase.

The study has real limits. It didn't track weight or smoking, few women used the skin forms, and micronized progesterone wasn't studied. Those limits are in the slides, because the full picture matters.

The bigger point: hormone therapy is not one treatment. Route, dose, type of progestogen, and your own health history all change the equation. And for women who can't or don't want to take hormones, there are effective nonhormonal options too.

The Selfority courses walk through all of it, hormonal and nonhormonal, so you can walk into your next appointment knowing the right questions to ask.

Save this for your next appointment.

Is the clot risk from hormone therapy about the hormone, or how you take it?

For more than twenty years, blood clots have been one of the biggest reasons women hesitate to start hormone therapy. That concern traces back to a 2002 trial (the WHI trial) that tested one estrogen pill, at one dose, in women who averaged 63 years old.

The finding was real. The problem is, it has been applied to patches, gels, different formulations (ex. estradiol), and doses that trial never studied.

There has long been a biological reason to question that leap. A pill passes through the liver first, and the liver responds by making more clotting proteins. Estrogen through the skin skips that step.

A new study of over 1 million women finally put pills, patches, and no hormones side by side. Part 2 breaks down what researchers found, including where the study falls short.

Follow so you don't miss it.

Yesterday was a beautiful Brooklyn morning to walk miles for menopause! Mpowered Menopause splashed the park in pink as Selfority moved with so many inspiring women and men to help fund menopause research.
Hearing from Linda B. Rosenthal before we walked was inspiring and advocating with Lisa McDonald | Menopause is heart warming. But, our biggest thank you goes out to our Selfority team members who supported us in person and virtual. Rachelle Kuramoto, Jamie Schlef Wilhelm, Hallie Cope, Gina Thomas, and NAO Wellness | Nikki Ostrower to name a few!
Community is so important and I love seeing it growing in person with so many amazing people.

Spent an hour last week with the graduate clinical nutrition students at UT-Austin talking all things menopause, and I am still smiling about it.

These are the people who will sit across from women in midlife for decades. They asked about bone, muscle, sleep, metabolism, and how to talk with patients when the evidence is still catching up. Sharp, curious, and ready.
Menopause barely shows up in most health training. Getting to help close that gap for the next generation of clinicians is the best kind of work.

Thank you for having me 🧡

The number that comes at the end of this one surprises almost everyone I tell it to. If you've had cancer, or you love someone who has, watch to the end. Breast cancer, alone, happens to 1 in 8 women. So, facts around menopause hormone therapy and cancer are important to know. Share this with the women in your life.
