A resident's condition can change quickly, and the earlier it's caught, the more options everyone has.

CFIM's physicians and APPs are connected to Florida's statewide Health Information Exchange, giving them real-time visibility into a patient's movement across hospitals, SNFs, rehabs, and home care.

Care coordination is considered the connective tissue of CFIM. A key differentiator of our dedicated, physician-led team. đź’Ş

We’d like to take a moment to recognize Allison Tucci, APRN, FNP-BC, for her clinical excellence within CFIM! 👏

Allison consistently shows exceptional clinical judgment. She knows her patients and their baselines well enough to catch changes in their condition early, and she acts on them right away.

She also brings the facility team into the process, helping everyone recognize the small changes that can make a difference in a resident’s care.

Thank you, Allison, for the proactive, engaged care you bring to this work every day! We are grateful to have you as part of the CFIM team.

It's Healthcare Quality Week! 🎉

In post-acute care, quality often comes down to the small things being done well every day.

For CFIM, that means providers rounding regularly, reviewing medications, managing chronic conditions, responding quickly when a patient’s condition changes, and staying closely connected with facility teams. These steps can help catch problems earlier and reduce unnecessary hospital visits.

To everyone on our team who makes that possible every day: thank you for all that you do!

Most people think of CFIM as physician coverage. But that’s only part of what we bring to a facility.

Through our Solution Medical Specialists team, CFIM facilities also have direct access to wound care, endocrinology, infectious disease, pulmonary, and podiatry. These specialists are post-acute dedicated providers.

Between specialty care, 24/7 on-call coverage, and weekly regulatory visits, CFIM gives facilities access to a broader medical team on site at the place they consider “home.”

Managing post-acute care across multiple facilities is much harder when every facility does things differently.

CFIM brings the same clinical standards and documentation practices to every facility it serves, giving MSOs and ACOs a more consistent care model across their network.

That consistency can make it easier to optimize quality and costs across multiple facilities.

Looking for a more consistent approach to post-acute care for your network? CFIM is here to help!

Non-dedicated medical coverage can create challenges that aren't always obvious at first.

Without a dedicated physician following the same patient population over time, facilities may experience:

• Less consistent follow-up between visits
• More reliance on documentation during handoffs
• Survey vulnerability, when care teams haven't coordinated and gaps go unnoticed until surveyors find them
• Changes in a patient's condition that are harder to identify early
• Overall, greater variability in day-to-day clinical processes

These issues don’t always appear in a service agreement, but they can influence the quality and consistency of patient care.

CFIM’s physician-led model is designed around continuity, with dedicated providers embedded in the facilities they serve to support patients, staff, and care teams over time.

Please join us in recognizing Ben Elliott on his promotion to oversee Care Coordination for Spectrum Medical Partners!

Many of our CFIM partners already know Ben through his work here, and as CFIM is part of the broader Spectrum Medical Partners network, they’ll now see him take that work into an expanded role across the organization.

As healthcare continues to move toward greater accountability for outcomes, Ben's role is focused on strengthening the connections between the hospital, post-acute facilities, our clinical teams, and community providers.

Through real-time patient identification, hospital and facility collaboration, transitional support, and close monitoring of high-risk patients, our Care Coordination program helps ensure patients don't get lost during transitions of care. Ben and his team are focused on identifying barriers early, improving communication across care settings, supporting safe transitions, and helping reduce avoidable hospital returns.

This expanded role reflects our continued investment in building a more connected post-acute care model and improving outcomes for the patients and healthcare partners we serve.

Congratulations, Ben! We’re excited to see you continue to grow this program and its impact.

For more than 20 years, CFIM has provided physician-led medical coverage to skilled nursing and assisted living facilities across Orange, Osceola, Polk and Seminole counties.

That kind of longevity doesn't come from switching providers every few months. It comes from physicians and APPs who are embedded in the facilities they serve, building real relationships with staff, residents, and families over time.

If your facility is looking for a medical coverage partner that's truly invested in knowing your residents, not just covering a shift, let's talk.

By 2060, the number of Americans age 65 and older is expected to nearly double: from 52 million to roughly 95 million.

Florida isn't waiting for 2060. The state already has one of the largest 65+ populations in the country, and post-acute demand is showing up in daily census numbers right now.

Physician-led, embedded coverage has never mattered more, and that’s precisely what CFIM is built for!

Patients recovering from a hospitalization sometimes carry more than the diagnosis that brought them in. Substance use disorder is one of the needs that can get missed in a standard post-acute care plan.

That's exactly why CFIM is rolling out substance use disorder services in the facilities we serve: integrated into the same physician-led care our teams already provide, not handled as a separate referral.

Facility leaders: if you have residents whose SUD-related needs haven't had a clear pathway before, let's talk about what this can look like at your facility!
