Home is where you join the weekly craft group, share a meal with friends, and know someone is always there for you.

Our Intermediate Care Facilities offer support with meals, personal care, and outings in the community, along with 24-hour nursing and medical care. The people we support can enjoy the things they love, with the care they need close by.

Every game brings opportunities to build strength, improve coordination, practice teamwork, and grow confidence. And with every high-five, shared laugh, and new skill, the people we support are building connections, staying active, and having fun along the way.
