Your mental health is part of your overall health. 💚

That’s the idea behind Integrated Behavioral Health (IBH) at Community Care Physicians.

CCP is proud to be one of the few fully integrated behavioral health programs in New York State, bringing behavioral health support directly into the healthcare setting so patients can receive care for both their mind and body as part of one connected team.

IBH can help with concerns such as anxiety, depression, stress, sleep, substance use, parenting, life transitions, lifestyle changes, and the behavioral side of chronic conditions.

And the data shows why an integrated approach matters:

📊 50–96% reduction in symptom distress within 6 months or less for patients receiving IBH care.

📅 94% of patients scheduled an initial visit, compared with 51% in specialty mental health care.

✅ 94% completed treatment, compared with 62% in specialty mental health care.

🚫 No-show rates were 5–15%, compared with 31% in specialty mental health care.

⏱️ For routine care, 77% of patients were scheduled within 2 weeks, while others received same-day or urgent appointments.

At CCP, Behavioral Health Consultants work alongside medical providers to provide evidence-based support, assessments, education, and interventions for mental and behavioral health concerns, while coordinating additional care when needed.

Your health. Your mind. One team, right here. 💚 🔗 Learn more: behavioralhealth.communitycare.com

This PA Week, we’re shining a spotlight on the incredible Physician Assistants who make a difference in our patients' lives every day.

From building lifelong relationships with patients and their families to helping patients recover after surgery, solving problems alongside patients, performing office-based procedures, and working as part of a collaborative care team, our PAs bring compassion, expertise, and a personal connection to everything they do.

We asked some of our CCP PAs what inspired them to choose this profession, what they love most about caring for patients, and what keeps them at Community Care Physicians. Their answers remind us that being a PA is about so much more than medicine. It’s about listening, building trust, empowering patients, and making a difference.

Join us in celebrating our CCP PAs and the many ways they care for our patients and communities. Happy PA Week! 👏

Have you ever wondered...

When should you start getting mammograms? How often should you be screened? And what does a typical breast cancer screening schedule look like?

The answers can depend on your age, personal history, family history, and other risk factors, which is why there isn’t always a one-size-fits-all approach to breast screening.

Throughout Breast Cancer Awareness Month, we’ll be sharing information from our Community Care Physicians breast health providers to help you better understand:

🎀 When to establish your breast screening baseline
🎀 How often screening may be recommended
🎀 What to expect during a typical screening
🎀 How your personal risk factors can affect screening recommendations
🎀 And more about the different tools used to evaluate breast health

💗 HAVE A QUESTION FOR OUR PROVIDERS?

This month, we want to hear from YOU! What have you always wondered about mammograms, breast cancer screening, breast health, or your personal risk factors?

👇 Drop your questions in the comments below!

Our CCP providers will answer your questions throughout the month and share information to help you feel more informed about your breast health.

📌 Follow along throughout October as we answer your questions and cover important breast health topics.

World Mental Health Day is October 10, and being heard matters.

This year’s theme, “Lived Experiences Heard: Real Voices, Real Change,” reminds us to listen to people with lived experience of mental health challenges and create space for every voice to be heard.

At Community Care Physicians, our Integrated Behavioral Health team believes that feeling heard, understood, and supported can make a meaningful difference in overall well-being.

💚 Join us Friday, October 9, at Wellness Way

We’re raising awareness about mental health one day before World Mental Health Day, with activities at 6 Wellness Way from 8:30 a.m. to 3 p.m.

Stop by the main entrance to:

🧠 Explore mental health resources and learn more about ways to support your well-being.

💬 Share what being heard means to you by adding a message to The Power of Being Heard wall. The wall will be available throughout the week leading up to World Mental Health Day. Your words could help someone else feel less alone.

💚 Wear green or a green ribbon pin on October 9 to show your support for mental health awareness and a community where every voice matters.

📱 Share your support on social media! If you participate or wear green, tag Community Care Physicians so we can help spread your message and share your support with our community.

Mental health is part of overall health. Whether you are seeking support for yourself, supporting someone you love, or simply learning more, taking the time to listen can be an important first step.

Let’s make space for real voices, real experiences, and real connection. 💚

Please join us in welcoming Leah Prescott, DNP, FNP-BC, to Saratoga Medical Associates!

A Saratoga Springs native, Leah has returned to the Capital Region after spending several years in the Washington, D.C., area. She is excited to care for the community she has always called home while providing whole-person adult primary care.

As a board-certified Family Nurse Practitioner, Leah is passionate about building lasting relationships with her patients. She believes great care starts with listening, educating, and partnering with each person to make informed health decisions.

Her interests include preventive care, cardiovascular health, and women’s health. She especially enjoys helping patients understand their health and creating individualized care plans that reflect their needs, values, and goals.

Leah’s connection to healthcare began at an early age, inspired by her mother, a nurse and case manager. She earned her BSN from SUNY Empire State College in Saratoga Springs, followed by her MSN as a Family Nurse Practitioner and Doctor of Nursing Practice from George Washington University.

Outside of caring for patients, Leah enjoys spending time with her family, exploring local nature trails, discovering new restaurants, and rowing. She also previously coached crew at Skidmore College! 🚣‍♀️

We’re thrilled to have Leah back in Saratoga and part of the Community Care Physicians family!

Welcome, Leah! We’re so glad you’re here! 🎉💙

Know your breasts. Know what’s normal for you.

Taking a few minutes each month to check your breasts can help you become familiar with how they normally look and feel, making it easier to notice changes that may need to be discussed with a healthcare provider.

A breast self-exam can be done in three simple positions:

🚿 In the shower: Use the flat pads of your fingers to gently feel each breast, checking for a lump, thickening, or other changes.

🪞 In front of a mirror: Look for changes in breast size, shape, contour, skin texture, dimpling, puckering, or changes to the ni***es.

🛏️ Lying down: Use your opposite hand to examine each breast in small, circular motions, covering the entire breast and underarm area.

If you notice a new lump, thickening, skin or ni**le changes, discharge, or any other change that feels unusual for you, talk with your healthcare provider.

🎀 All women are at risk for breast cancer. Everyone should know how to check their breasts, pay attention to changes, and follow recommended breast cancer screening guidance from their healthcare provider.

Remember: A breast self-exam is not a substitute for routine clinical breast exams or recommended breast cancer screening.
