My daughter Gabbie Scudder, on her own initiative, purchased beverages, snacks and Tylenol to pass out at the peaceful #blm protest in Wash DC yesterday. My husband Jeff accompanied her with his camera. Here are some of my favorite shots that he captured of this important, and humbling, moment in history. I am proud of my daughter for being one of many lights in our Capital city yesterday. May is Mental Health Month, so I thought I’d touch on a somewhat delicate topic related to our brains, our emotions and pain.

As this diagram from Louis Gifford’s research paper shows, we receive information from our environment and our body tissues via our nerves. This information, in the form of electronic impulses, travels to our spinal cord and then from there may be sent up to the brain. Multiple areas of the brain process these signals and make a decision as to whether or not these signals indicate danger to the body. If the brain perceives danger, the result will be pain. Even though it is produced in the brain, all pain is real and causes us to behave in any number of ways, usually to move to get away from the perceived source of danger.

Our mental health can have a significant impact on that process and will either ramp up or ramp down our sensitivity to these potential danger signals, particularly in situations when pain is long-standing (i.e., more than 3-6 months). If fear, anxiety, sadness, or stress is the mental status of the day (or week, month or year!), body pain may very likely be the output from the brain.

Conversely, happy, joyful, contented and grateful mental states tend to ramp down the nervous system, and may even prevent messages being sent from the spinal cord up to the brain in the first place.

One of the best ways to keep our mental health in check is through exercise. Our brain unlocks a number of “feel good chemicals” when we move, that can stabilize and even enhance our mental health state.

And recent research (lots of it) is supporting the positive therapeutic effects of education about pain neuroscience. So, there you go!

I am blown away by the fact that I am able to reach hundreds of people globally each week. In the month of April I was able to present movement sessions and posture tips to nearly 350 employees from the US and the UK, Mexico, Brazil, India, Sweden, Hungary, Spain, Portugal, France, Italy, Poland, Estonia, Slovakia, Slovenia, Latvia and Croatia.

I was even able to use some of my language skills to speak with the employees in Guadalajara, and received some really great feedback! “Muchas gracias Doctora, mi espalda se lo agradece!”
I’m feeling so blessed that my career has brought me to this point. 🥰

I love learning and have been on a journey these past few years relearning the physiology of pain.

So much has changed in this area of science since I first learned about it 30+ years ago in PT school, and even in the past 12 years since I returned to grad school for my Doctor of PT degree.

Pain is not simply damaged or injured body tissue. It’s information from our nerves, our environment, our beliefs, our experiences, and our memories, all working together to help our brains decide whether or not the body is biologically in danger. If the brain decides yes, the body is in danger, then boom, there’s the pain. And when that happens, all the 9 or so areas of our brain that are part of this matrix are unable to function normally. This may be why it’s hard to move normally or think clearly when we have pain. The brain is “high jacked”. When we have long term pain, well, the word “enslavement” was used here by Adriaan Louw, PT, PhD, pain researcher and instructor of the Pain Neuroscience Education course I’m currently taking.

The good news is, just learning about how pain works, in combination with movement, can make a world of difference. And that’s what I am striving to do: to help people MOVE and KNOW that they don’t have to be held captive by pain.

Are you taking movement breaks every hour? Even more impressive!

Two go-to movements that I often recommend, after a movement assessment, to people who are having low back and neck discomfort in sitting. These movements can also be beneficial for people in standing in some cases. They are movements that our spines rarely get the chance to do functionally, but need for maintaining the health of those spinal soft tissue and bony structures.

1) Standing backwards bend: feet hip width apart, hands supporting the top of the pelvis (just below the low back), lean backwards are far as you feel comfortable going, and after just 2-3 seconds, rerun to upright. You may feel pressure or compression in the middle of your back or even off to one side while leaning backwards, but as long as this discomfort subsides after you return to standing, you are probably safe to do several more repetitions.

2) Chin tuck and look up: slide your head back as if you are backing away from a kiss or a touch to your face that you do not want (social distancing!!). Then, when you have pulled back as far as you can (your chin will be close to your throat), drop your head back and look up at the ceiling. Relax your shoulders and stay for 1-3 seconds then return to starting point. Again, you may feel some pressure or mild discomfort in the center of you neck, and that is fine, as long as it subsides soon after you return to the starting position.

Welcome to Scudder Physical Therapy!

Telephyzio by Scudder Physical Therapy is the first and only telehealth-based pain relief delivery model in Loudoun County, Virginia. I help patients with a variety of musculoskeletal pain issues--low back, neck, shoulder, elbow, knee, hip, even ankle/foot and wrist/hand problems. Patients in Virginia and Maryland are evaluated and treated online, when and where they want, in the comfort of their own homes, or even in their work places. Each session is individualized to the patient, as a 60- or 30-minute one-on-one session with me, a Doctor of Physical Therapy. Using the McKenzie method to evaluate how movement effects pain behavior, I then coach patients to use simple movements and posture changes to relieve pain and prevent recurrence.

I am the owner and sole practicioner of Scudder Physical Therapy. The majority of my 30-year career practicing physical therapy has been spent in Northern Virginia. I have gained a wide range of experiences in outpatient orthopedics using various treatment methods. I became Certified in the Mechanical Diagnosis and Therapy (MDT or the McKenzie method) in 2003 and have been using that approach to patient care very successfully ever since. It is one of the most efficient and evidence-based approaches to evaluate the source of musculoskeletal pain, which then leads directly to a movement-based treatment protocol. Empowering patients to treat themselves, with my coaching, is the cornerstone of the success of this method (https://www.mckenzieinstituteusa.org/method-patients.cfm). To maintain my certification, I attended twice yearly continuing education courses and seminars, from which I always return with more knowledge and skill in relieving patients from pain.

While working as a school-based PT for Loudoun County Schools, I served for four years as the head coach for the Broad Run High School JV and Varsity volleyball teams. I also coached with local club teams over a 10 year period, and served as the Movement and Conditioning Coach for Virginia Elite Volleyball Club in 2008 as well. I love working with volleyball players!

I have been blessed with opportunities to help people around the globe. While living temporarily in Morocco I worked with disabled orphans. During a two-year stay in South Africa I earned an Honorary Physiotherapist license, working with cancer patients in a public hospital. More recently, I have travelled to Peru twice on week-long medical mission trips with Mechanical Care Everywhere, a MDT-focused humanitarian mission underserved Peruvian patients at free clinics are treated using the MDT method only, empowering them to care for themselves.

I enjoy an active lifestyle including biking, kettlebell training and playing volleyball. In my quiet moments I enjoy hiking and travelling with my husband and our rescue dog Rio, visiting our three grown daughters and two Cavapoo granddogs whenver we can.
