💉 Vaccinations are NOT required for membership.

• $100/month per adult
• $59/month per dependent when at least one adult has a membership
• $100/month per dependent without an adult membership
• One-time enrollment fee: $100/person
• Family enrollment fees are capped at $200

The simple answer is: DPC and health insurance are two different things.

🩺 DIRECT PRIMARY CARE (DPC)
DPC is a membership for primary care services.

At Collier Family Medical, your monthly membership gives you access to your primary care practice without traditional office-visit copays.

Your membership includes:

✅ Primary care office visits
📱 Direct text messaging with your provider as a routine part of the practice
🧪 Routine lab work once a year
❤️ Chronic care
🚨 Urgent care
📞 After-hours care for urgent matters

🏥 HEALTH INSURANCE
Health insurance is coverage for healthcare expenses according to the terms of your insurance plan.

Depending on your plan, this can include things such as:

• Specialist care
• Hospital care
• Emergency services
• Imaging
• Surgery
• Other covered medical services

💙 THEY CAN WORK TOGETHER.
DPC does not replace your health insurance.

Instead, you can use DPC for your primary care relationship while maintaining your health insurance for healthcare services covered under your insurance plan.

Think of it as:

DPC → Primary care
Insurance → Covered healthcare expenses outside your DPC membership

Your insurance plan still determines what it covers, your deductibles, coinsurance, network requirements, and other costs.
