SHC Medical Partners was formed in 2003 in response to the need for skilled nursing facilities (SNF) to have more frequent, on-site medical services. Today, we are a premier transitional medical services company that brings personalized medical care to those patients we serve. We currently provide medical services in 7 southeastern States and are very excited to be expanding our coverage area and

adding new service lines! Our dedicated and compassionate team is committed to building a trusting and rewarding relationship with those we care for and work with by providing our services with integrity and serving with humility. is moving toward outcomes-based reimbursement models and access to affordable insurance coverage. The demand for medical providers is further increased by the emergence of 71.3 million aging baby boomers and resulting medical advances that enable quality living through management of chronic diseases. The Primary Care provider remains the sole trusted authority for coordinating quality care, yet the shortage of physicians is at a critical level. The article, “Direct Primary Care: Key Issues and the Future” by Dave Chase, stated this primary care physician shortage is estimated to be 35,000 to 44,000 by year 2025 (Forbes, July 2013). We believe that these realities create an historic opportunity for SHC Medical Partners which is to respond to this growing need by providing transitional medical care, a model that is deeply embraced by our medical providers. These principles apply to our interactions with patients and their families, our fellow Stakeholders at SHC Medical Partners, and the multitude of care professionals that team with us to deliver services. We focus on delivering quality transitional medical care beginning with coordinating discharges from a hospital stay, proceeding to managing medical care while at rehabilitation facilities – with a goal of enabling our patients to ultimately reside in a setting they choose with proper medical oversight. Our medical providers participate in the full spectrum of medical service delivery including acute care, emergency care, rehabilitation care and primary care. We ARE changing the delivery of medical care and enhancing the quality of life to those we service!

This is what Abode Care Partners in action looks like: Our team member Audrey Avitabile is making a huge difference in patient care at the Rosewalk assisted living facility in Indianapolis.

Audrey was recently recognized for her exceptional performance in providing high-quality care to her resident patients. Several members of her organization have commended Audrey and the value she has brought to her work with each patient.

Chief Medical Officer Arif Nazir says, “With her engagement, the facility has shown continuous improvement with a significant decrease in hospitalization rates.” Audrey has been a vital contributor to the day-to-day care provided. She and the rest of the Indianapolis team have made strides through understanding the current needs of the operation and catering to the workflows and practices so those areas could receive the most individualized attention.

Great job, Audrey! 👏 We’re excited to see Rosewalk and its patients continue to improve and thrive.

Welcome to Letisha Blair, new Director of Starting Point Nursing Services (a division of BrightSpring Health Services), which provides remote nursing support for Abode Care Partners. Letisha joins BrightSpring with 25 years of contact center experience supporting multiple lines of business.

Letisha has held a variety of leadership roles and has a proven track record of successfully driving quality, efficiency, and engagement including operations at AT&T, Alight, and Oak Street Health. Notably she opened the Oak Street Charlotte call center as the day one employee in 2019. Letisha partnered with key stakeholders to grow the operations to over 400 clinical and non-clinical team members during the last 5 years. During that time, the inbound call center was transformed into a contact center supporting inbound/outbound calls (triage, appointment scheduling, RX refills, general messaging), SMS texting, web/email submissions, and special initiatives around STARS.

The work of the centralized contact center greatly reduced the workload of field teams and clinicians allowing them to dedicate more quality time to patients ultimately improving clinical outcomes. Letisha has been recognized for her leadership, driving balanced performance, building a great place to work, and was the recipient of the 2022 Gold Stevie Award for Customer Service team of the year.

Letisha resides in Charlotte, North Carolina and enjoys spending time with her family, travel, sporting events and live music.

Welcome to Caitlin Jones, new Associate Vice President of Business Development and Sales Leader for Abode Care Partners (ACP), a division of BrightSpring Health Services.

Caitlin comes to ACP with 12 years of experience in the long-term care, senior living, and home-based care settings.

She has held leadership positions in operations and business development within many rehabilitation-based companies including Genesis Rehab Services, EmpowerMe Wellness, and Infinity Rehab. Most recently, Caitlin served as the National Sales Director for Infinity Rehab where she was responsible for business development, strategy, and revenue growth nationwide.

Caitlin is a Speech-Language Pathologist by background. She received her Bachelor of Science from Saint Louis University in Communication Sciences and Disorders and her Master's degree from Kansas State University in Family Studies and Human Services.

Caitlin lives in Seattle, Washington with her two children.
