10/23/2026

The Centers for Medicare & Medicaid Services Advancing Chronic Care with Effective, Scalable Solutions Program (ACCESS) is active through 2036.

However, there's one date quickly approaching and an important addition to the ACCESS Model:
🗓️ October 31, 2026, is the application deadline for the ACCESS cohort starting on January 1, 2027. Cohorts continue on a rolling/quarterly schedule.
🗓️ April 1, 2027: This cohort is notable because it's tied to the expansion of ACCESS to include new tracks for heart failure, chronic obstructive pulmonary disease (COPD), substance use disorder, to***co cessation and chronic musculoskeletal (MSK) pain (follow-on period).

Also, read the Updates to the ACCESS Program: Four Additional Condition Tracks Added to the Advancing Chronic Care with Effective Scalable Solutions (ACCESS) Model from the Center for Connected Health Policy at đź”—https://bit.ly/4yFdWTp

The CMS ACCESS Program will be with us for a while! Please share your interest in participating in ACCESS, along with any questions or concerns, via a short form. We’re not asking this idly; we hope to develop resources and assistance based on your feedback. Completing the form should take no longer than ⌛ 3 minutes.

The Rural Health Information Hub and the NORC Walsh Center for Rural Health Analysis have released a new AI in Rural Healthcare Toolkit. This free resource supports rural healthcare organizations as they consider whether and how to adopt artificial intelligence.

The toolkit covers the opportunities and limitations of AI in rural settings, offers guidance on selecting tools and responsible data use and shares program models for clinical decision support, telehealth and remote patient monitoring, clinical documentation, billing and scheduling.
