What happens to a patient after they leave the hospital? 🏥

Yesterday, led by our Chief Strategy Officer, Aisha Alizai, MS, PA-C, our team at Synergy Medical Group met with MedStar Washington Hospital Center and GBMC HealthCare to address this exact question. The transition from hospital to discharge is one of the most vulnerable points in a patient’s journey. Through initiatives like our advanced heart failure program, we’re ensuring patients receive seamless handoffs and dedicated support every step of the way—from the hospital, to the SNF, and home.
