✨SELF-TREATMENT FOR PELVIC FLOOR OVERACTIVITY✨
➡️The pelvic floor muscles can become sensitive and overactive for many reasons. When this occurs, it can contribute to pelvic/abdominal/low back/hip pain, as well as bladder problems like urgency/frequency and difficulty voiding, bowel problems like bowel urgency and constipation, and many other symptoms. 😣
💕The best step to improving overactivity is to connect with a pelvic floor therapist for individualized examination and treatment. Self-treatment to the pelvic floor muscles can often be helpful if that is something you and your provider want to try.
To do this:
✅Use lots of lubricant on your thumb or finger, and position yourself comfortably where you can reach your pelvic floor with ease.
✅Insert your finger into the va**nal or re**al ca**l gently, just a little bit. gently press down and identify if your tissues feel sensitive. If they do, take slow long breaths. ✅Gently contract and relax your muscles. Then, re-check and see how they feel.
✅Move to another area at the same depth. If in the va**na, be cautious not to mash the urethra (so avoid pressing at 12 o’clock). ✅Repeat this process. Then move a little deeper, and repeat.
🥰Keep your discomfort low while doing this. The goal is to gently desensitize the structures. If it’s not comfortable, be sure to talk with your PT to find out if it’s the most therapeutic option for you.💜

🥂Happy thanksgiving from all of us at SPH!
We spend a lot of time spreading awareness about pelvic health problems, and recognizing where care often falls short. But today, we focus on everything we are grateful for in the pelvic health world. The truth is, we have come a long way in the past 5-10 years. Pelvic rehab is now a common, well-known specialty. The number of pelvic rehab providers has exploded!! PelviCon was launched 2 years ago, and brings over 600 providers together annually for inspiration, community and elevation of the standard of care people deserve! (And registration for PelviCon 2024 opens TOMORROW!)
We still have a long way to go… but for today, we are grateful! We’ll keep doing the work to move pelvic health forward!! Thank you for being here, and being you!

Tailbone pain is a pain in the butt!😣(Ha…ha…)
No, seriously. This little area can cause quite a bit of difficulty for people. The coccyx (tailbone) is composed of 3-5 bony segments and sits at the base of the sacrum. It is a major attachment for several muscles within the pelvic floor.
Tailbone pain can be caused by:
☀️ Childbirth
☀️ Pelvic floor muscle overactivity
☀️ Long-term butt clenching
☀️ Falls
☀️ Sitting in the same position for long stretches of time (you are built for movement!)
☀️ …and a lot more.
Annnndd, tailbone pain often doesn’t come from the tailbone. In fact, it can be referred from structures like the:
✅ pelvic floor muscles
✅ obturator internus muscle (deep inside the hip)
✅ a**l sphincter
✅ low back, sacrum and hip
✅ gluteus maximus
If you’re struggling with tailbone pain, there is help available. ✅ Start working on moving—avoid sitting for very long stretches of time. ✅ Aim to focus on lengthening and relaxing your pelvic floor muscles. ✅ Pay attention to any gripping patterns you have around your pelvis (especially your butt!), and actively work to let go. ✅ And, send us a DM if you need more help!

Oh hi! Time for an introduction!
I’m Dr. Jessica Reale- a pelvic health physical therapist and the leader of Southern Pelvic Health in East Cobb/Marietta and Midtown, Metro Atlanta. A few things to know about me:
👋 I’m a Colorado- native, but I’ve been living in the south for 16 years! We moved to Atlanta in January 2015 and absolutely love living here!
🤓 I’ve been obsessed with pelvic health, and have been in this specialty since 2008! I love everything about it- especially helping people with complex cases!
🤗 My husband Andrew and I started SPH in 2019 because we believed that people with pelvic health problems deserved the absolute best, evidence based care! We built the practice to be a space fully dedicated to pelvic health and we have an amazing team who believe in that mission.

Pelvic health problems can be so common after birth… and they’re often very treatable. Some of the most common problems are:
✅ Bladder leaks, urgency, and increased frequency
✅ Bowel difficulties (constipation, bowel leaks)
✅ Pain during seggs (especially with painful scars or after cesarean births)
✅ Prolapse and laxity in the pelvis
✅ Low back and pelvic girdle pain

Yep, water. It can seem so simple, but getting adequate fluid can make a huge difference for your pelvic health. What does it impact?
✅Constipation: Your colon recycles about 70% of the fluid that enters it. If you are not getting enough fluid during the day, it can be common to experience dry and more firm stool. Constipation can lead to a whole slew of pelvic health problems-- worsened pelvic pain, worsened bladder problems, and more!
✅Bladder Leakage: Not getting enough fluid can lead to a concentrated urine. This type of urine is more likely to contribute to urinary urgency and increased frequency. It can also make it more difficult for the bladder to hold.
✅Decreasing UTIs: Your body needs to get rid of any bacteria that may enter the urethra, and flushing the urethra regularly with dilute urine is a great way to do that! When a person is dehydrated, they can be more likely to experience urinary tract infections (which can worsen most pelvic health symptoms and feel pretty yucky!)
✅Painful S*x: Yep-- dehydration can lead to dryness everywhere... even in the va**na! Decreased moisture may increase discomfort during s*xual activity.
⭐️⭐️⭐️So... how much fluid do we all need? It's not a simple answer. The truth is that it may vary based on your body type, exercise habits, lactation status, the weather...and more! It can be helpful to start by getting around 1/2 of your body weight in ounces-- meaning that if weigh 160 lbs, 80 oz of water per day can be a good starting place. Then, you can adjust the total up and down based on how you feel!

Yesterday, I had a little bit of a late start so enjoyed a beautiful walk at the with my dog Teddy. We loved seeing the sun as it was rising over the river, and Teddy definitely got his feet covered in dirt. Starting off the day with movement can bring so much peace and clarity to a busy day.

These reasons don’t even begin the cut it! The pelvis is amazing, and such an integral part to so many functions in the body. The muscles of the pelvic floor connect to our bowel, bladder and s*xual health… as well as our breathing, and even our voice! The pelvis enables us to transfer force from our legs to trunk. There are connections with posture, and so so so much more!

💥 Returning to swimming & starting new things! 💥
When I was in high school, I swam on the swim team. I have always loved swimming as a form of exercise and movement. I loved the way I could focus on my breath, focus on moving and gliding through the water, and I left the way it made my body and muscles feel.
In college, I worked as a lifeguard, and switched into more recreational swimming. After grad school, I competed in my first (and only!) triathlon.
Then, I had children. Two pregnancies, two C-sections. And I didn’t swim for exercise in a pool for six years, until this year.
I have started, it was hard. I was slow, my breath was heavy. But it has gotten better. New things can always be hard. But now that I am nearly 2 months in, I’m feeling stronger. Swimming further. Breathing easier.
There is hope if you haven’t done something in a while. You can get back to movements you love. And that’s what I needed to say today. 🥂

💥PelviCon 2023… it’s hard to know even where to start! It is an absolute privilege and blessing to be able to serve the pelvic rehab world in this way! and I have a deep passion for bringing all pelvic rehab providers together— building community, elevating the standard of patient care, changing the way things are typically done. Patients deserve our BEST.
This past weekend 625 pelvic rehab pros came together to learn, rekindle old friendships and start new ones, and reignite our passions for pelvic health. We laughed (pretty hard!), we cried, we shared in each other’s joys and hardships— we lifted each other up in ways I’ve never seen in our profession. We proved that WE are better together.
I am truly grateful for this community, this movement, this fire that is growing. PelviCon is here to stay. The future of pelvic rehab is bright! It starts with us! ❤️

🥰One month from today…. We’ll be back, getting ready for 625 pelvic rehab providers to arrive at the second annual PelviCon conference in Atlanta, GA!!
We started PelviCon with and because we believe that patients deserve the BEST, most evidence-based, up-to-date, and ground-breaking treatment available.
We believe the pelvic rehab providers need to come together to do better and be better for our patients. We believe that a rising tide lifts all ships, and we needed to become the force to lift the tide of pelvic rehab!

⭐️Meet the Obturator Internus ⭐️
➡️This muscle is a deep hip external rotator and shares an attachment with the pelvic floor muscles (forming the side wall). It has fascial connections around the urethra as well! It is an important muscle for pelvic floor health and hip stability, and this little muscle can become problematic for some people!
Pain in the OI can feel like:
✅ Deep hip pain
✅ Pain radiating to the front, side or back of the hip
✅ Buttock pain or tailbone pain
✅ Bladder or bowel dysfunction (ie difficulty emptying the bladder, spraying of the bladder stream)
✅ Pain with s3x
✅ Pain radiating to the back of the thigh
✅ Pain or pressure in the va**na or re**um
😬It can be a tricky muscle, and I can’t tell you how many times I have pressed on this muscle and have heard, “Yes, that’s the pain that I’ve been feeling!!”

Yep. You heard it here. Having a diagnosis of prolapse or diastasis re**us abdominis does not mean you can no longer lift weights, do CrossFit, run, or anything else. If you’ve been led to believe that, it may be time to get another opinion. The key is loading your muscles and tissues appropriately- improving your management of pressure, and helping you progress in exercises in a way that is serving to you.

✨DO YOU KNOW WHAT HAPPENS WHEN YOU EAT?✨
Digestion is often an ignored process until it becomes a problem. Since my gallbladder problems, and recent gallbladder removal surgery, I’ve become even more acutely aware of the process.
Here are some fun digestion facts:
✅ Digestion begins in your mouth, and saliva plays a role in beginning to break down food.
✅ When you eat a meal, think of eating, or even smell food, the colonic meal response helps to push contents within and through your system
✅ Contents within the re**um are frequently sampled by the sensitive nerves in the a**l ca**l. This lets you know if you have any gas that needs to 💨 or if you need to find a toilet.
✅ Your pelvic floor muscles are super important for regular bowel function. They help you hold in gas and stool, but also, coordinate to promote full emptying!
Did you learn something new? Did you know that pelvic floor PTs and OTs can help with bowel problems like incontinence and constipation?

🌸 April is Cesarean Birth Awareness Month 🌸
As a mama to two beautiful girls born by Cesarean, it’s a topic that hits me very close to home. I’ve heard less sensitive things along the way indicating that people who had C-sections “got the easy way out” or “didn’t really try” or “didn’t actually give birth” or people putting reasons behind the C-section, like indicating we “didn’t prepare enough” or “had anxiety so things couldn’t open” or many many other things (often unhelpful, untrue, and unkind).
🌸 So here’s what I do know. Birth is hard— no matter how it happens. Birth is unpredictable, and things can change regardless of preparation. Anyone who births, has birthed. All birth can be empowering. All birth is incredible. Everyone who births can benefit from appropriate rehab, and needs support.
And kindness and empathy always wins.

💡Pelvic floor overactivity is so common (in fact, I think it may be more common than true weakness!)
⭐️Overactivity or tension in the pelvic floor muscles can cause symptoms like:
➡️Pain in the hips, low back, ge****ls, abdomen, or anywhere really around the pelvis.
➡️Bladder urgency, increased frequency or difficulty emptying the bladder (and guess what, it can cause leakage too!)
➡️Constipation or difficulty emptying stool
➡️Pain related to s3xual intimacy
➡️And guess what… it can also cause things like a sensation of core weakness, tension in the upper back, cramping pain in the abdomen, and more!

We are so thrilled to be rolling out this program again . I’ve taught pelvic health coursework for 7 years, and every time I teach classes, there are so many clinicians attending who are heading back to be the only pelvic health practitioner at their practices. ✨Endometriosis: a Multidisciplinary Approach✨

Join Dr. Ken Sinervo of Center for Endometriosis Care and Dr. Jessica Reale and Dr. Sara Reardon on Wednesday March 8 at 7pm CST/ 8pm EST for this 90-minute class. [ Please note that this class is intended for health care professionals ]

Dr. Ken Sinervo is the Medical Director of The Center for Endometriosis Care () and is known worldwide not only as an expert endometriosis surgeon, but more importantly, as a humble, compassionate doctor who truly cares for all of his patients from every walk of life and every corner of the globe. We are honored and thrilled to have him present on the diagnosis and medical management of Endometriosis.

He will be joined by Dr. Jessica Reale () and Dr. Sara Reardon (.va**na.whisperer) who will share strategies to treat the musculoskeletal, bowel and bladder, and nervous system components that often accompany endometriosis pain.
This class will provide tools to:
✅ Screen and identify patients with Endometriosis
✅ Understand current research for best medical and surgical practice for people with Endometriosis
✅ Expand their rehabilitation treatment options to better serve their patients with Endometriosis

Wow- we had such an incredible time at the APTA Combined Sections Meeting in San Diego! Some of our highlights:
❤️ Dr. Jessica and Dr. Brooke ( ) got to present on intrare**al balloon training for bowel disorders.
❤️ Dr. Jillian (.watson.dpt ) was sworn in as the director of the early professionals special interest group.
❤️ Lots of fun hanging with our SPH team (.watson.dpt ) and connecting with sooooo many amazing physical therapists
❤️ Enjoyed connecting with our partners- Nicole & Jesse, and meeting their sweet baby Clay!
Now, back to the office!
