This week, Celebrating Life Community Health Center proudly recognizes the dedicated professionals who help our patients navigate their health care journeys with compassion, advocacy, and support.

We are especially grateful for our Enhanced Care Management (ECM) Lead Care Managers, Medication-Assisted Treatment (MAT) team, and Peer Navigators, who work tirelessly to connect individuals and families with the care, resources, and guidance they need to thrive.
