Meet Samuel, a Korean war veteran who's receiving services from our Care Management Organization (CMO) and Veterans Outreach teams.

Samuel has always been someone who looks out for others and his daughters describe him as active, independent and humble. John, one of our home care nurses, quickly bonded with Samuel saying “He’s a great guy and we can talk about anything. Service, family, fishing, being fathers of daughters.”

Samuel’s physical therapist, Christopher also loves talking with Samuel, saying good conversation can be a good medicine. “We talk and laugh, and that takes his mind off the pain of moving.”

After Samuel was discharged from a Long Island hospital, he was enrolled into our Care Management Organization and was quickly linked with our Veterans Outreach team. CMO Clinical Care Manager Seth, made the first connection to Samuel, and being the mother of a veteran herself, knew how to connect with Samuel. “I’m a nurse,” says Seth, “so I initially look at a diagnosis. But I’m also looking for who you are as a human being and what you did in your life. That’s the best part of connecting people to VNS Health’s Veterans program—finding out who these people are along with the branch they served, any war time they saw, and what it all means to them. Very powerful.”

Thanks to the support from his friends, family, and our CMO and Veteran Outreach teams, Samuel was able to get the care he needed. Our veteran liaison team even held an in-home ceremony, honoring Samuel for his service with a certificate of Appreciation.
