Ask your workplace health vendor when their last onsite clinic ran. Then ask what time of day it ran.

Most programs are built around a 9 to 5, because that is when the vendor is staffed. Nights, weekends, and rotating shifts get an eligibility line and not much else. They also tend to be the people who need it most. NIOSH has documented what shift work does to sleep and cardiometabolic risk.

What real coverage takes:
✔ Clinic hours that follow the shift schedule, including overnight
✔ The same screening panel for every shift, never a reduced one
✔ Enough notice that people can plan around it
✔ Participation tracked by shift, so you can see who is being missed

If your program has never run at 2 a.m., part of your workforce has never really been offered it. Worth fixing before next plan year.

A standard physical can come back completely clean and still miss what matters most.

October is Sudden Cardiac Arrest Awareness Month, and the hard part of that is how often these events arrive with no warning anyone noticed. Blood pressure and cholesterol describe risk. They do not show what is already happening inside the arteries.

A coronary calcium score does. It is a quick, low-dose CT scan that measures calcified plaque, and it can find real disease in someone whose other numbers looked fine.

Why that matters for a workforce:
→ It surfaces risk in people who feel healthy and have no symptoms
→ It gives an employee something concrete to bring to their provider
→ It is most informative in the age band that holds much of your senior staff

Whether it is right for any one person is a question for their physician. Whether your people can get to that conversation easily is where we come in.

For a self-insured employer, the expensive diagnosis is almost never the early one.

Caught at a routine screening, a condition is a manageable claim and an employee who stays at work. Caught two years later, it is a very different line on your report, and often someone who is out for months.

You cannot know in advance which employee that will be. Which is why screening only makes sense across a whole population, not one person at a time.

What a screening program changes:
✔ Conditions surface while they are still straightforward to treat
✔ Fewer emergent, high-cost episodes late in a plan year
✔ Less time lost to extended absence and return to work
✔ A baseline you can trend year over year

We will not attach a dollar figure to that. Every population is different. Which way it moves is not really in doubt.

Curious what screening participation could look like at your organization? Let's talk.

Today is National Mammography Day.

If you have been meaning to schedule yours, here is the nudge. And if you know someone who takes care of everyone else in her life first, send it to her. She is usually who this reminder is for.

Fifteen minutes on a calendar. Worth rearranging a day for.

Your provider can tell you what is right for you and when.

A benefit announced in November is a benefit nobody remembers in March.

Open enrollment is a narrow window. People hear about a screening benefit once, in a packet, alongside every other decision they are making that week. Then they go back to work and it goes quiet.

What actually determines whether they use it happens over the next eleven months:
→ Does anything remind them, or was the packet the whole plan
→ Is there a date on the calendar, or just an eligibility line
→ Does using it cost them half a day
→ Does their manager treat it as normal

Onsite care handles most of that by putting a real date on a real calendar, somewhere people already are. The reminder becomes an event instead of an email.

Worth a conversation about what that could look like on your calendar next year.

Most departments meet NFPA 1582 on paper. Fewer run 1583 at all.

The two standards do different jobs. 1582 covers the medical evaluation: what gets examined, what conditions matter, and how a member is cleared for duty. 1583 covers the rest of the year, the health-related fitness program that keeps members ready between evaluations.

Where it usually breaks down:
✔ Annual evaluations that slip to every other year when the budget tightens
✔ Medical findings that never connect back to a fitness plan
✔ No consistent provider, so nobody is tracking trends over time
✔ Volunteer and part-paid members held to a looser standard than career staff

Chiefs, safety officers, and union leadership: running evaluations at the station is the practical way to keep both standards on schedule. Happy to walk through what that looks like for your department.

Screening guidelines only work if people can actually get to the screening.

Breast Cancer Awareness Month tends to focus on the guidance. Know your risk, know the recommended age, do not skip the appointment. All true, and all of it assumes the appointment is reachable.

For a lot of your workforce it barely is. A half day off, a drive, a waiting room, and a schedule that bends around everyone else in the house first.

Most of that is the part an employer controls:
→ Does taking the time feel permitted, or does it feel expensive
→ Do people know what their plan already covers
→ Does the reminder come once a year, or when it matters
→ Can care come to them instead

Early detection changes outcomes. Access is what lets it happen.

Fire Prevention Week is aimed at the public. It is also a good week to look at what your crew carries home.

The IAFF and NIOSH have documented elevated rates of certain cancers among firefighters, tied to the toxins and exposures of the job. Departments have gotten better at the fireground half of that: decon, gear discipline, clean cab practice. The other half is screening, and that is the part that tends to run on good intentions.

What a consistent program looks like:
✔ Comprehensive screening on a set annual cadence, not after someone has a scare
✔ On or off duty, at the station, so nobody loses a shift for it
✔ Bloodwork, skin, cardiac, and respiratory handled in one visit
✔ Provider follow-up that actually closes the loop

Chiefs and union leadership, if your screening program is informal, this is the week to put it on paper. Let's talk.

If you want an onsite program running in January, October is when it gets decided.

By the time enrollment materials go to print, the vendor questions behind them have already been answered. So the employers whose people will be screened in Q1 are settling four things right now:

→ The goal. Participation, claims, absenteeism, or duty of care. Pick the one you will be measured on.
→ The population. Who is eligible, and who gets missed if you only cover headquarters.
→ The shifts. Second and third shift are where most programs fall apart.
→ The sites. One location or twelve, and what it takes to run them consistently.

None of that is a long conversation. It just has to happen before the window closes.

HR and benefits leaders, if a Q1 launch is on your list, this is the month to scope it. Let's map it out.
