Continuum Care Hospice is dedicated to
treating each person as an individual with unique
needs and goals.

When you have a life-limiting illness, Continuum Care Hospice brings compassionate care and support to you, so you can live in comfort and dignity where you are most at home, close to those you love. A life-limiting illness does not
have to impair the way you live your life. Your
professional hospice team works with you, your
physician, and those close to you to customize
a plan of care that meets your specific medical,
social, emotional and spiritual needs.

The theme of Spiritual Care Week for 2026 reflects the Japanese art of Kintsugi, where broken pottery is repaired with gold, silver, or platinum lacquer. Rather than hiding the cracks, they are highlighted – transforming something that once was broken into something more beautiful and valuable.

Join Continuum Care Hospice as we honor the sacred work of chaplains and spiritual care providers — those who walk beside the brokenhearted and offer healing, not by fixing, but by being present.

Being on hospice doesn’t mean you can never go to the hospital. ❤️

But before heading to the ER, call your hospice team first whenever possible. There may be options you didn’t know were available, and going to the hospital can sometimes affect how your hospice care and coverage are handled.

That phone call can help you understand your choices before making a decision.

Did you know you should call hospice before going to the ER?

As we gather to celebrate with family, good food, and fireworks, let's take a moment to honor the true meaning of today. Thank you to the veterans, active-duty troops, and their families who sacrifice so much so we can live free. May we never take our liberty for granted. Happy Independence Day! ❤️🤍💙

The final 24 hours of life can be one of the most emotional and uncertain times for families.

You may notice changes that feel frightening or unexpected—long periods of sleep, changes in breathing, cool hands and feet, less responsiveness, or moments when your loved one seems to drift between worlds.

These changes are often a natural part of the body’s final journey.

Knowing what to expect can’t take away the sadness, but it can help families feel a little less afraid and a little more prepared.

If you’re walking through this season right now, I hope this information brings some comfort and reassurance.

💗 Your loved one may not be able to respond, but your presence still matters.

💗 A hand to hold, a familiar voice, and simply being there can be more meaningful than you realize.

What was one thing you wish someone had explained to you about the final days of life?
