Palliative care isn’t about waiting for the decline. It's about helping patients LIVE well with chronic disease. 💙
Better outcomes start with earlier conversations, clear plans, and communication across the entire continuum of care.
Return-to-Hospital (RTH) numbers are rarely just about a lack of medicine. Often, they reflect a lack of planning and communication. When no one knows what matters most to the patient, or what the plan is when their condition changes, the default can quickly become sending them back to the hospital.
That’s why every patient needs more than a diagnosis and a medication list. They need a story, an explanation, and a care plan that everyone understands. Primary Care Providers (PCPs), social workers, case managers, hospitals, facilities, patients, and families all need to be part of that conversation.
Don’t wait for a crisis to start communicating. Know the patient. Know their story. Know their goals.
When the entire care team understands the “why” and is aligned around what matters most to the patient, we can improve outcomes, reduce unnecessary returns to the hospital, and provide truly patient-centered care. 🌹
