Three symptoms, one cause almost nobody connects.
You wake up around Заm and can’t fall back asleep.
For the ten days before your period, your patience is gone and small things set you off. And your cycle keeps showing up a day or two early.
On their own, each one gets brushed aside.
Together, they’re a recognizable signature of low progesterone.
And this isn’t only apparent in perimenopause. Low progesterone can show up in your 20s and 30s too, after a stressful season, after a baby, or for no
obvious reason at all. If you have a cycle, you can have this pattern.
Progesterone is your body’s steadying hormone. It quiets the brain, supports deep sleep, and softens the anxious edge that builds premenstrually. It rises in the second half of your cycle, so when it runs too low in those weeks, the 3am wake-ups and the shorter fuse aren’t a personality flaw. They may be the chemical consequence of low progesterone.
Here’s the part that changes everything: the timing is the tell. A symptom that shows up on the same stretch of your cycle, month after month, is pointing at a hormone that drops on that same schedule. You don’t have a sleep problem and a mood problem and a cycle problem. You have one rhythm causing havoc at the same point every month.
Identified correctly, it’s also treatable. Supporting progesterone through the back half of your cycle can settle the sleep and steady the mood that low progesterone pulls apart.
If this is your month, every month, that repetition the clue worth following.
✅ Book a hormone evaluation and get the cyclical pattern behind your symptoms mapped, not brushed off.

You filled the prescription. You’re consistent with it.
You followed the instructions.
So why do you still feel like something’s off?
This is one of the most frustrating experiences in hormone therapy. You’re doing your part, but the results don’t match what you expected. And the instinct is to assume the hormones aren’t right for you.
Before changing the protocol, it’s worth asking a different question: is your body actually getting what you’re putting in? We have seen several patients recently with sub-optimal hormone levels even on seemingly adequate doses.
Several factors can reduce how effectively your body processes hormone therapy:
→ Chronic stress increases cortisol, which can interfere with hormone receptor sensitivity
→ Poor gut health can impair absorption, especially with oral formulations
→ Liver congestion affects how hormones are metabolized and cleared
→ Nutrient gaps like magnesium, zinc, and B vitamins can limit the enzymes your body needs to convert and use hormones properly. This is why we do things differently Oasis-obgyn and why follow-up labs matter as much as the initial prescription. And why symptom tracking between visits gives us the context that labs alone can’t.
✅ If you’re on hormone therapy and still not feeling the way you expected, the next step may not be a new prescription. It may be understanding what’s standing between you and the one you’re already on. Let’s take a closer look. Hormone optimization isn’t once a year and done.

One of the most common reassurances women hear before a hysterectomy is that keeping their ovaries means their hormones will stay the same.
For some women, that holds true. For many, it doesn’t.
The uterus shares a blood supply with the ovaries.
When it’s removed, circulation to the ovaries can decrease, and that reduced blood flow may cause them to produce less estrogen, progesterone, and testosterone over time.
The timeline varies. Some women notice changes within months. Others experience a more gradual shift over years. But the symptoms are consistent:
→ Fatigue and mood changes that weren’t there before
→ Vaginal dryness or discomfort that appeared post-surgery
→ Decreased desire with no clear explanation → Weight changes that don’t match eating or activity patterns
→ Sleep disruption that started after recovery
These symptoms are often attributed to aging or stress. Rarely does anyone connect them back to the surgery.
A post-hysterectomy hormone evaluation can show whether your ovaries are still producing at the levels your body needs, and whether targeted support could address what shifted.
✅ If new symptoms appeared after your hysterectomy and no one’s connected them back to the surgery, a post-hysterectomy hormone evaluation can show whether your levels have shifted. We can help. Reach out today to get started.

For many women, the hormone therapy conversation never happened but because the warning scared them out of ever asking.
A black box warning is the most serious safety label the FDA can place on a medication. Seeing one on a hormone product made millions of women assume the risk was simply too high. And for just as many healthcare providers, it became a reason to never bring it up.
Twenty years of hot flashes managed with antidepressants. Sleep disruption treated with sedatives. Mood changes chalked up to stress. All while the treatment that could have addressed the root cause sat behind a label that made it feel off-limits.
In February 2026, the FDA removed those warnings after concluding the risk had been overstated for the women and formulations most commonly used today.
This doesn’t mean hormone therapy carries zero risk. Every treatment decision involves weighing what you stand to gain against what you need to monitor. But the starting point for this conversation is no longer a warning label. It’s your symptoms, your history, and your goals.
If you closed the door on HRT years ago because of what you were told, it may be worth opening it again.
If the black box warning kept you from ever exploring hormone therapy, the conversation looks different now. Schedule your per menopause or menopause, hormone replacement consult and let’s evaluate what’s appropriate for your situation.

You noticed the changes first. The sleep issues. The brain fog. The weight that stopped responding to what always worked. But when labs were drawn, nothing was flagged.
That’s because blood levels are the last thing to move. Your tissues, your receptors, your nervous system all respond to hormonal shifts well before a lab value crosses a clinical threshold. You’re not imagining it. Your body recognized it before the test did.
Hormone care that only reacts to abnormal labs is always behind. Evaluation should start with what you’re experiencing, then use labs to confirm and guide treatment.
✅ If you’ve been feeling off but your labs haven’t caught up yet, we can help you connect the dots.
