Dissolving filler without ultrasound is basically playing hide and seek… but you’re blindfolded. 🙈

Filler doesn’t magically sit where we think it is. It migrates, layers, encapsulates, and hides in places you wouldn’t expect.

With ultrasound, we can actually see the filler, guide the dissolving process, and treat precisely where it is.

Blind dissolving = guessing.
Ultrasound-guided dissolving = knowing.

If you’re a patient, it’s okay to ask:
“Are you using ultrasound while you dissolve?”

Because dissolving everything around the problem doesn’t fix the problem.

Dissolving filler isn’t just about having ultrasound in the room.
It needs to be used in real-time while you are dissolving.

Ultrasound-guided dissolving means we can:
✔ visualize the filler
✔ watch the enzyme interact with it
✔ confirm we’re treating the correct plane
✔ stop when it’s gone and protect everything else

If you’re a patient, ask this:
“Will you be using ultrasound to guide the dissolving in real time?”

If the answer is no… you deserve better.

Why Ultrasound Matters When Assessing DAO

The depressor anguli oris (DAO) plays a major role in lower facial expression, specifically pulling the corners of the mouth downward. In aesthetic practice, the DAO is commonly targeted for neuromodulation to soften a downturned smile or “sad mouth.”

However, not all DAO anatomy is textbook, and one of the most clinically relevant variations presented in this case involves aberrant or blended attachments with the buccinator muscle.

This is where ultrasound becomes critical.

Understanding the DAO–Buccinator Relationship:
• The DAO normally originates from the mandible and inserts into the modiolus at the oral commissure.
• The buccinator, a deeper facial expression muscle, forms the muscular wall of the cheek and plays a role in mastication and oral competence.
• In some patients, DAO fibers partially blend with or abnormally attach into the buccinator, rather than following a clean, superficial course.
• This variation is not reliably predictable by surface anatomy alone.

Why This Anomaly Matters Clinically:
When a buccinator-DAO anomaly is present. blind injections based on landmarks may:
• Miss the true DAO belly
• Diffuse toxin into the buccinator
• This can result in:
• Asymmetric smile
• Oral incompetence or cheek biting
• Speech or chewing changes
• Under-treatment or over-treatment of the DAO

The DAO is not a “set it and forget it” muscle.
When a buccinator attachment anomaly exists, ultrasound is the only way to see it before you treat it.

👉 If you’re treating the lower face without ultrasound, you’re treating a guess, not anatomy.
👉With ultrasound instead of assuming anatomy, you are confirming it.

STOP SCROLLING, INJECTORS 🚨

This is the perfect example of why ultrasound is becoming non-negotiable in aesthetics.

Today’s case: a spoon-shaped pocket of misplaced lower-face filler.
Yes , we could palpate it. But we had no idea how big, how deep, or how irregularly shaped it truly was… until we scanned it. 🔍🔥

Here’s what ultrasound revealed:
• A large, elongated filler pocket
• A shape we couldn’t completely appreciate by palpation alone
• Poorly integrated product contributing to lumps + heaviness
• Borders impossible to assess accurately by touch alone

Once mapped, we performed precise ultrasound-guided dissolving, targeting ONLY the problematic areas:
✔️ Cleaner contour
✔️ Less enzyme used
✔️ No over-dissolving
✔️ Safer + more predictable correction

Injectors — these lower-face cases are where US truly shines.
Palpation will tell you something’s there…
but ultrasound will tell you exactly what you’re dealing with.

Stop guessing. Start seeing.
Ultrasound is the advantage you didn’t know you needed.

I’m incredibly honored to have spoken at the FANA Conference this October. Sharing space with so many dedicated CRNAs, SRNAs, and advanced practice leaders reminded me why our profession is so powerful and why our voices matter now more than ever.

Thank you, , for the opportunity to contribute to the conversation and to highlight the ways we can continue pushing our profession forward.

CRNAs have always been innovators, problem-solvers, and patient advocates. Whether in the OR, at the bedside, in the classroom, in research, or in emerging areas like ultrasound-guided practice, we continue to show what advanced nursing leadership looks like.

Our future depends on staying engaged, staying visible, and staying united. When CRNAs lead, nursing moves forward.

“3-Year-Old Filler Dissolve”

Sometimes a fresh start is the real glow-up.

This patient came in with heaviness around the nasolabial folds, and the culprit wasn’t age…it was 3-year-old filler that was weighing the tissue down.

Using ultrasound-guided dissolving, we located the old product, safely dissolved it, and immediately saw the mid-face lift and softening she’d been missing. Removing the weight of old filler often restores more natural contour than adding more.

Moral of the story: You don’t always need more filler, sometimes you need the right plan. 💡
Ultrasound = precision.
Precision = better outcomes.

✨ For Practitioners: The Art (and Controversy) of Dissolving ✨

Teaching practitioners how to properly evaluate, scan, diagnose, and dissolve filler isn’t about undoing someone’s work,
it’s about restoring anatomy, protecting tissue health, and giving patients a better outcome.

If you want to elevate your skill set, make evidence-based decisions, and feel confident managing complications, mastering dissolving is a must.
