If you're a man over 40 and you've been feeling tired, gaining belly fat, losing muscle, or noticing changes in your s*x drive, it's tempting to blame testosterone.

And yes, sometimes low testosterone is part of the problem. But it's not always the whole story.

I see men who come in convinced they need TRT, when what we really need to do first is look at their sleep, metabolic health, cardiovascular risk, medications, and overall health.

One thing I tell my patients all the time is that we don't chase numbers. I don't treat a testosterone level. I treat the person sitting in front of me.

That's why these five things are worth investing in because taking care of your health after 40 isn't just about testosterone, it’s about maintaining your strength, protecting your heart, having energy, and staying healthy enough to enjoy the years ahead.

And when testosterone replacement therapy is appropriate, we discuss it as part of a comprehensive plan, with proper evaluation and monitoring.

If you've been wondering whether your symptoms are related to low testosterone, that's exactly the kind of conversation we can have at Downstate Adult Health.

After more than 25 years in healthcare, one thing I've noticed is how often people put off addressing their health concerns, and there are so many reasons for it.

Sometimes we're just too busy dealing with everything else in life. Sometimes we convince ourselves that being tired all the time, gaining weight, sleeping poorly, or having little interest in s*x is just part of getting older, and sometimes, if we're being honest we're afraid of what we might find out.

I get it, it's much easier to tell yourself you'll deal with it later, especially when you're still managing to get through your day and do everything you're supposed to do.

But just because you've gotten used to feeling a certain way doesn't mean it's something you should automatically accept. Not every symptom means there's something seriously wrong, but if something keeps coming back or is starting to affect your quality of life, I think it's worth finding out why.

I'm curious, is there something about your health you've been putting off addressing, and what's been holding you back?

IV therapy and supplements can both be used to support your nutritional needs, but they work differently.

Neither is automatically better. What matters is understanding what your body needs and choosing the right approach based on your individual health and nutritional status.

Before starting IV therapy or supplements, talk with a qualified healthcare professional to determine what’s appropriate for you.

If you’ve tried IV therapy or supplements before, share your experience in the comment.

Here are some signs to pay attention to:

1. CHANGES IN YOUR PERIOD
Your periods may become:
• More frequent or less frequent
• Heavier or lighter than before

2. MOOD CHANGES
You may notice:
• Increased irritability
• Anxiety

3. HOT FLASHES & NIGHT SWEATS
Sudden feelings of intense heat, sweating, flushing, or waking up drenched at night can occur during the menopause transition.

4. BRAIN FOG
Having trouble concentrating, remembering things, finding the right words, or staying focused?

5. VAGINAL & SEXUAL HEALTH CHANGES
6. Hormonal changes can contribute to:
• Vaginal dryness
• Discomfort during s*x
• Changes in s*xual desire
• Urinary symptoms

These symptoms are common but that doesn’t mean you have to simply live with them.

Fall is one of my favorite seasons. 🍂

The crisp mornings, the leaves starting to change, the sun coming up on my morning walks… I love all of it.

My hands? Not so much. 😂

This is what Raynaud’s looks like for me. As soon as the temperature starts dropping, my fingers can go from red to blue/purple to completely white. They get numb, uncomfortable, and sometimes painful.

Raynaud’s phenomenon is basically an exaggerated response of the blood vessels to cold temperatures or even emotional stress. The small blood vessels constrict, blood flow temporarily decreases, and you get the lovely assortment of colors you see here.

Keeping warm, avoiding sudden temperature changes, wearing gloves before your hands get cold, and avoiding things that further constrict blood vessels, like ni****ne, can help. For people with more significant symptoms, there are medications that can help too.

If you have Raynaud’s too, tell me in the comments. I’m curious how many of you deal with this, and if there’s enough interest, I’ll do a follow-up on what actually helps. Because once the temperature drops, the struggle is real. 😂

I hear some version of these questions all the time.

- “Is this just because I’m getting older?”

- “My labs were normal, so why don’t I feel normal?”

- “Should I be worried about this, or am I overthinking it?”

Sometimes the answer is simple, and sometimes it isn’t. A lab result matters, but it doesn’t exist in a vacuum. Neither does a symptom.

I want to know what changed, when it changed, what else is going on, what your health history looks like, what medications you take, how you sleep, how you eat, what your risk factors are, and yes, what your labs show.

That’s the difference between looking at a number and looking at the whole person. And no, I don’t think every symptom means something is wrong. But I also don’t think feeling exhausted, gaining weight, losing your s*x drive, sleeping terribly, or simply saying “I don’t feel like myself anymore” should automatically be written off as getting older.

Sometimes the most important part of the conversation is simply asking, why?

Your questions are worth asking. And you deserve more than “everything looks normal” when you know something has changed.

The symptoms may be common, but that doesn’t mean they should automatically be dismissed.

Fatigue gets blamed on being busy, brain fog gets brushed off, sleep changes get ignored.

Your libido shifts, and vaginal dryness gets treated like something you just tolerate. These are symptoms and they are worth paying attention to.

Instead of assuming this is just part of getting older, pay attention to what has changed and how long it has been happening.

What’s one symptom you wish more people understood instead of dismissing as “just getting older”?
