What does a ThriveMore at Home Wellness Coordinator do?

Think of this dedicated RN as a knowledgeable partner for your aging-in-place journey. Your Wellness Coordinator gets to know you, helps you create a personalized wellness plan and checks in over time. If an injury or illness occurs, they can help coordinate services, communicate with providers and advocate for your needs.

You do not have to navigate a complicated health system alone. With ThriveMore at Home, you have someone who knows your goals and is ready to help.

The word is getting out about Continuing Care at Home programs such as our own ThriveMore at Home! The New York Times recently published an article explaining the structure and benefits of these programs for seniors who do not wish to move into a brick-and-mortar Life Plan Community.

Journalist Joanne Cleaver spoke with ThriveMore's President and CEO Reed VanderSlik as she wrote this article, consulting him for context and accuracy.

We are proud of ThriveMore at Home, which has more than 40 members just a few years after we launched in the Triangle. It provides peace of mind and wellness support for members, drawing on ThriveMore's 75-year history of serving older adults in North Carolina.

One of the best times to make a plan for future care is before you need it.

ThriveMore at Home members join while they are healthy and independent. That gives them time to think through their preferences, build a relationship with their Wellness Coordinator and put support in place without the pressure of a fall, illness or emergency.

Planning ahead means future decisions can reflect what matters most to you, rather than what must be decided quickly in a crisis.

Start the conversation today. Your future self may thank you.

Why healthy adults should plan ahead before a health crisis occurs, instead of assuming there will be plenty of time to make decisions later on.
