The "Coordinated Care Model" was the primary focus for Session 3 of the Central Valley Health Network Leadership Academy's inaugural cohort.

CVHN enlisted the leadership of Dr. Nick Stark, Co-Founder & Co-Director, Emergency Medical Innovation Collaborative (EMIC). Dr. Stark expanded on the concept of coordinated care, challenging participants to view patient care gaps not as individual failures, but as opportunities to strengthen the systems that support patients and communities. He emphasized that effective leaders identify where people become disconnected from care, close communication loops, build partnerships across organizations, and create systems that make the right actions the default. The discussion reinforced a core Leadership Academy principle: Lasting change is driven by leaders who listen first, co-create solutions with those closest to the work, remain agile while staying focused on outcomes, and transform successful innovations into sustainable practices. By strengthening collaboration and improving systems, leaders can create meaningful impact across their organizations and communities.

Session 3 of the Central Valley Health Network Leadership Academy's inaugural cohort focused on the "Coordinated Care Model."

As part, CVHN called upon the expertise of Cristina Rendon, Director of Health Education & Wellness, Camarena Health. She demonstrated how Camarena Health's coordinated care model improves patient outcomes by connecting patients to the right services and engaging the entire care team. In Cristina's words, "Success is not only making the referral. It is helping the patient connect with care." 💚
