❓Is your ICU ready for flu season❓

Even a moderate flu season can bring patients with life-threatening respiratory failure.

For ICU teams, preparation includes recognizing when severe influenza progresses to ARDS and being ready to deliver the therapies these patients may need.

In our latest blog, “Flu Season 2026–2027: Is Your ICU Ready?” we explore:

🔹 This season’s outlook and how it compares with last year
🔹 How severe influenza can progress to ARDS
🔹 The role of prone positioning for eligible patients

WHO IS USING AUTOMATED PRONE THERAPY? 🤖🫁

We've spent a lot of time discussing manual prone programs. Now we're curious about the other side.

If your hospital uses the Pronova-O2™ Automated Prone Therapy System, we want to hear from you!

Specifically: Are you using IntelliDerm® Powered Skin Protection during prolonged prone sessions?

Unlike passive foam materials at the patient-support interface, IntelliDerm® is engineered to actively address the four root drivers of skin breakdown:
🔥 Heat
💧 Moisture
↔️ Friction
↕️ Shear

📊 ANONYMOUS CLINICIAN POLL:
A. We manually prone
B. We use the Pronova-O2™
C. We’ve used both manual and automated proning
D. We’re currently evaluating automated prone therapy

Provide the letter of your response in the comments below

30% DEVELOPED PRESSURE ULCERS. WHERE? EXACTLY WHERE YOU'D EXPECT. 👤📉

In one mechanically ventilated ICU cohort undergoing manual prone positioning, approximately 30% of patients developed prone-related pressure ulcers1.

Among the most commonly affected locations?

📍 The face (including the cheekbones and chin)
📍 The chest & sternum
📍 Iliac crests & knees

That matters because prone positioning creates a unique biomechanical challenge: We are taking anatomical areas never designed to sustain prolonged load and asking them to do exactly that. For hours. Often repeatedly. In critically ill patients who cannot reposition themselves.

❓ So here's our question for bedside clinicians:

What is your hospital using to protect the FACE during prolonged prone positioning?
🔹 Prophylactic foam dressings?
🔹 Fluidized positioners?
🔹 Specialty prone pillows?
🔹 Scheduled 2-to-4-hour head turns?
🔹 Something else?

👇 ICU nurses, RTs, and critical care teams: tell us what’s working (and what isn't) in the comments!

💡 Looking for automated technology engineered to mitigate pressure, friction, shear, and microclimate buildup during prone positioning?

• The Pronova-O2™ automates rotation and prone positioning to relieve sustained interface load.
• IntelliDerm® provides active microclimate management to protect vulnerable skin interfaces.

PRONING SHOULDN'T HAVE TO COME WITH A PRESSURE-INJURY TRADEOFF 🫁🛡️

Prone positioning can be lifesaving in appropriately selected patients with moderate-to-severe ARDS.

But prolonged prone therapy creates a well-documented skin-integrity challenge.

The answer isn't to prone less. The answer is to make prone therapy better.

✨ Better positioning.
✨ Better access.
✨ Better pressure redistribution.
✨ Better microclimate management.
✨ Better protection from friction and shear.
✨ Less dependence on repeated manual intervention.

At Turn Medical, we're continuing to ask a simple question: How can technology help clinicians deliver evidence-based prone therapy while better protecting the patient receiving it?

• The Pronova-O2™ automates prone positioning.
• IntelliDerm® provides powered management of heat, moisture, friction, and shear.

And we're not finished innovating.

Turn Medical recently had the privilege of presenting at the AACN Greater Phoenix Chapter meeting and evidence based medicine came up in the conversation. One of the clinicians mentioned that it was only 30-40 years ago that evidence-based medicine was something new and that much of the practice of nursing was not evidence based at all (i.e, q2 hour turning).

This conversation was the impetus for a new series that we are starting called The Clinical Turn. This series will focus on new and pioneering data as it is published that provides new insights into the care of ARDS, pressure injuries and the transplant process.

Our first segment comes from a recent article published in Nursing in Critical Care by Gay and colleagues titled "A Bundle of Interventions to Prevent Pressure Ulcers During Prone Position in Adult Patients with Acute Respiratory Distress Syndrome: Results of a French Stepped-Wedge Randomized Controlled Trial".

To summarize, 54.8% of patients randomized to the prevention bundle developed a new pressure injury by Day 7 compared to 63.9% randomized to usual care. There was no statistically significant reduction. This outcome was in spite of the use of the prevention bundle in 91.8% of this cohort.

Ultimately, the study demonstrated that the bundle of interventions did not lead to a significant reduction in the frequency of new pressure injuries in ARDS patients treated by prone positioning. However, due to the number of study limitations presented, the results of this trial should not lead us to interpret that prevention bundles don't work. Rather, this trial demonstrates the variation and complexity of pressure injury prevention in proned ARDS patients and informs as to improvements in a new trial design.

You can access the publication here: https://pmc.ncbi.nlm.nih.gov/articles/PMC12163295/

Our hope is to do our part to ensure that evidence-based medicine is the standard of care.

MANUAL VS. AUTOMATED PRONE THERAPY: WE WANT TO HEAR FROM THE BEDSIDE 🩺

This isn't a theoretical question. We want direct feedback from the ICU nurses, respiratory therapists, and critical care teams actually proning patients at the bedside.

📋 IF YOU MANUALLY PRONE:
• How is your current skin-protection program performing?
• Are you satisfied with your pressure injury outcomes?
• How many staff members are required to safely perform a turn?
• How challenging is maintaining a strict prevention protocol over 16+ hours?
• Are hospital-acquired pressure injuries (HAPIs) still occurring despite high compliance?

⚡ IF YOU USE THE PRONOVA-O2™:
• How has automated prone therapy changed your clinical workflow?
• How has it impacted the number of staff required to position and rotate patients?
• Are you using IntelliDerm® Powered Skin Protection?
• What differences in skin integrity and efficiency have you observed compared to manual proning?

👇 Manual and automated prone clinicians—we’d love to see both perspectives in the comments below! 💬

Clinical innovation only gets better when bedside clinicians share what is actually happening at the point of care.

WHEN BUNDLE COMPLIANCE ISN'T ENOUGH 📉📑

For years, the critical care consensus for reducing hospital-acquired pressure injuries (HAPIs) during prone therapy was simple: Build a better manual bundle and enforce compliance.

A recent study put that assumption to the test. Researchers evaluated an intensive manual prevention bundle in mechanically ventilated ARDS patients undergoing prone positioning:

🔹 Prophylactic eye and eyelid care
🔹 15° Reverse Trendelenburg body inclination
🔹 Specific anatomical head and thorax cushions
🔹 Scheduled head rotations every 4 hours
________________________________________

📊 THE RESULTS: • Protocol Compliance: 91.8%

• Pressure Injury Outcome: 54.8% of intervention patients developed a new pressure injury by Day 7 vs. 63.9% in the control group (p>0.05).

The Takeaway: Near-perfect ex*****on of an aggressive manual bundle failed to produce a statistically significant reduction in pressure injuries.
________________________________________

❓ WHY DID MANUAL BUNDLES FALL SHORT?
Because manual repositioning and passive foam cannot continuously eliminate the root mechanical and thermodynamic drivers of skin breakdown:

1️⃣ Unrelieved Interface Pressure: Sustained loading on prone anatomical contact points.
2️⃣ Microclimate Deterioration: Accumulation of heat and moisture at the skin surface.
3️⃣ Friction & Shear Forces: Unavoidable movement stresses created during manual turns.

We cannot ask bedside nurses to simply "try harder" with static, passive tools. We need a fundamental change in how we support prone patients.

💡 ENGINEERED TECHNOLOGY FOR CRITICAL CARE

Turn Medical offers an integrated solution to take the physical burden off the bedside team:

• The Pronova-O2® automates prone positioning and continuous bilateral rotation up to 65°.
• The IntelliDerm® Powered Skin Protection System actively manages heat, moisture, friction, and shear at the patient- support interface.

Stop relying solely on manual interventions to solve a mechanical and thermodynamic challenge.
