I say this every👏single👏day👏

Patients are so often dismissed because they “don’t have BAD periods”. (Or ~ONLY~ have “bad periods”.)

They are not given the appropriate diagnosis of endo because they don’t have bad periods. Because their symptoms look more like constipation. Or bladder pain. Or pain with s*x.

Or they’re told that birth control will fix whatever the bad period is from- spoiler it doesn’t fix the CAUSE.

Often, when I first bring up endo- patients say “oh no I don’t have that- my periods are totally fine”. Great! Love that for them! And it still can VERY MUCH be endo.

If I could spend every single day screaming this to anyone who would listen- I would. So here’s the weekly reminder that endometriosis CAN look simply like “bad periods” but it can be so many other things too💛

PERIOD.

If you are going to have surgery with someone, for suspected or known endo, it should always be with the intention of treatment.

Every time you go in for surgery, it’s an inflammatory event. Going in to look/ diagnose and NOT remove everything- is potentially giving you answers for a cause of symptoms but definitely increasing your global inflammation. Especially if they leave endo. And definitely not the “gold standard” of care.

There is SO much more to say in this, and there’s definitely some nuance to the conversation, but simply put this.is.the.goal.

I almost never ever ever think kegels are appropriate during pregnancy.

Raaaarely are straight “kegels” someone’s homework- for any symptom or diagnosis.

Even more rarely are they helpful during pregnancy I’ve had two patients EVER- doing “kegels” while pregnant (and they were profoundly weak with complete incontinence).

Most of the time, it’s quite the opposite. MOST people’s pelvic floor is tight and overactive during pregnancy.

Often, people will call to start pelvic therapy while pregnant, and their goal is to “strengthen the pelvic floor for birth”…

The job of the pelvic floor during labor and birth is to relax and ~get out of the way~. Kegels make the pelvic floor tighter. They make the pelvic floor less mobile and therefore make it harder for the pelvic floor muscles, especially posteriorly, to relax and lengthen to help get that head out.

So PLEASE, stop doing kegels during pregnancy✨

AND have a pelvic PT assess your pelvic floor multiple times throughout your pregnancy- as things are constantly changing😘

Endometriosis doesn’t always show up as the stereotypical “horrible period cramps.” Sometimes, it looks a lot like a gut problem🫠

Honestly, if I have a female patient present with ANY unexplained bowel issues- I’m questioning endo, almost immediately.

OFTEN I am able to recognize endometriosis in people when they present with IBS like symptoms, including:

💛Chronic constipation- not going for 3-5 business days, struggling to go, feeling backed up, or never feeling fully empty.

💛The constipation to diarrhea rollercoaster- sometimes people will experience BOTH, and go back & forth between each.

💛Bloating- can often be a side effect of endo. Possibly from the inflammation, GI/systemic sensitivity, constipation & back up, pressure change in the bowels, MCAS, SIBO (there’s also research on potential “seizure like activity in the intestines”). All of these things can make endometriosis look a lot like simply ~bloating~.

If your “IBS” symptoms are UNEXPLAINED, cyclical, worsen around ovulation or your period, come with significant abdomen/ pelvic/ bu****le pain, you have painful bowel movements (in the abdomen or bu****le), or just never seem to fully make sense, it may be worth bringing up endometriosis.

Maybe it’s IBS, but sometimes it’s endometriosis 🫶

This is the least common presentation that I see in my clinic. It happens, but I think most of the time this is caught after the person is diagnosed based on other more common symptoms. Once the person starts investigating everything else they’re struggling with, they may find this.
