Before your appointment, you’ll receive an Information Request Form and be asked to provide your medical records. This allows our doctors to review your information ahead of time and determine if any additional testing may be needed.

For your records, please provide the summary reports of your scans within the last 3 months and bloodwork from within the last 3 weeks.
