“My main focus from the time I got here was to get everyone to accept who we are – a community hospital serving a poor community,” said Gustavo Valdespino, President and Chief Executive Officer of Valley Presbyterian Hospital. “To me, it doesn’t matter whether somebody has a lot of money or not enough money. Everyone is somebody’s family member and deserves to be treated with utmost dignity and respect.”

Congratulations to Ryan Xiong for receiving Employee of the Month for May 2024.

Ryan Xiong has been a part of Partners in Care Foundation for a little over two years. Ryan’s ability to turn complex data sets into actionable insights is truly unparalleled. His work has been instrumental in driving the Private duty business forward and making data-driven decisions a reality. Examples of his work are Vendor performance with visualizations to identify trends immediately. Another example is a calculation report that displays hours on vendors to help the Private duty team make data driven decisions to not go over the allotted hours. Ryan has also identified the profits and losses from declined services to see the net effect. The following fantastic qualities that sets Ryan apart is: great collaborator, maintains open communication, provides technical support, organized, helpful, patient, a pleasure to work with, and quick to respond to inquiries made from team members. His role is integral in the Private Duty Billing and invoice process on a monthly basis. We appreciate Ryan and his hard work, as well as his unwavering commitment to our mission and team. For Nurses Week, we are spotlighting staff nurses and their stories. Today, we are highlighting Marisol Amaral.

Marisol Amaral is a Registered Nurse for Partners’ Short-Term Services and Supports (STSS) Face to Face program.

Healthcare has always been her passion. When she was a teenager, she dreamt of going to Medical School and becoming a doctor. She started out as a medical assistant and then spent four years working in Urgent Care, and it was there she saw firsthand the positive impact of nurses in patient care.

She obtained her nursing degree and worked nine years bedside in critical care. During her time as a critical care nurse, she noticed the lack of knowledge patients had for community resources available to help them heal.
She joined Partners in 2022 and resonated with the mission of “aligning social care and health care”. Her responsibilities in the STSS program include determining eligibility of individuals to enter Community Based Adult Services (CBAS) centers, so vital to their community living success.

She knows the great impact of CBAS centers as her grandmother living with dementia was a member of a CBAS center for over seven years. At CBAS centers, they stress the importance of work to maintain personal independence, avoiding unnecessary and costly institutionalization.

Marisol reflects, “I feel I am truly making a difference in our underserved community helping with opening access to these important service sites.”

We are celebrating Nurses Week with Staff Spotlights of Nurses. Today, we are highlighting Janelle Honore.

Janelle is a Nurse Care Manager for Partners Home and Community Based Alternative (HCBA) Waiver Program.

Janelle shares, "Working with Partners has allowed me to meet individuals in their unique circumstances, aiding them in reaching their health-related goals through effective education and personalized care strategies".

She has over 24 years of experience as a Registered Nurse, with broad experience working in Medical Surgical, the Emergency Room, and Ambulatory Care.

Early in her career, Janelle recognized the importance of a holistic approach in healthcare by working with home health agencies and intermittent care facilities for adults with disabilities.

In 2017, she accompanied a L.A. based medical missionary team to Ethiopia to provide nursing services at the newly constructed Negeli Arsi Hospital and its surrounding villages.

Her career as an HCBA case manager launched two years later in 2019. Community-based nursing has proven to be a dynamically impactful setting to care for patients by addressing their physical, mental, and spiritual wellbeing.

For Nurses Week, we are spotlighting staff nurses and their stories.

Today we are highlighting Liliana Pena. Liliana Pena is a Nurse Care Manager for Partners’ Multipurpose Senior Services Program (MSSP) in Kern County. From a very young age, she was drawn to the health and medical field. She spent eight years as an ultrasound technician before deciding to pursue a career in nursing with encouragement from her aunt and cousin, who are nurses.

During her externship with a hospital, she worked closely with the aging population and found her passion for caring for older adults. Liliana’s husband was closely connected to a former Partners’ supervisor, and after she graduated, they recommended her for a position within the Multipurpose Senior Services Program team.

“At Partners, I get to interact with a group of people that often feel ignored or overlooked. I have found it to be rewarding to get to be part of a team that helps seniors stay in their homes and avoid nursing institutions.”

Liliana has been with Partners for four years now and enjoys being able to help and provide services to better care for the aging population.

May is Older Americans Month!

Where would we be without our seniors?

So much of this agency’s focus is on caring for older adults. Our waiver programs – MSSP and HCBA – are important tools that help the fragile remain in their home. Our care transitions programs enable individuals to safely leave an institution and return to their residence with a lessened likelihood of having to return due to health-related social needs. The community wellness team teach seniors how to prevent falls, safely exercise, and manage chronic conditions. HomeMeds makes sure that medication errors are caught so don’t lead to falls, or medical complications. And the Long Term Service and Supports team are guardian angels who aid in myriad ways – from helping find the homeless a place to live, to making sure there is food in the refrigerator and that a ride is available for the next appointment with a doctor

None of this would be possible without the Partners in Care Foundation staff. It doesn’t matter whether you are in direct social care or are part of the agency’s support functions. Every role is important as each allows the agency to provide crucial services to those in our care.

On behalf of every older resident in our care, thank you for the excellent skill and attention you bring to work with you each and every day. Without you, there are people in our community whose lives would be harder.

Yvette Perez has been with Short Term Support Services (STSS) for 3 years. She currently works with patients from Providence St Johns Medical Center. Yvette is a knowledgeable, dedicated, and caring individual who takes pride in serving her patients.

She is a team player, someone who makes herself available to others. Yvette assists new staff by sharing her best practices and tips on organization and time management. She is always willing to share resources with her team helping others grown and be more successful. She is a great example of what a strong case manager should be. She is reliable, compassionate, and thorough in her communication. Yvette makes sure she gets to the root of the issue and provides quality resolutions for our clients.

Yvette is always willing to help patients schedule appointments so that they connect with a doctor after discharging from the hospital to ensure that they are continuing their care. Partners Multipurpose Senior Services Program (MSSP) attended the Give Big Kern Event last weekend. GIVE BIG KERN is a major event in Kern County that offers the opportunity for community members and organizations to support a cause they care about.

Partners' MSSP serves senior residents in Kern County with Medi-Cal and living with complex chronic health conditions. MSSP services support independent living at home, and help seniors avoid unnecessary and costly nursing home placement. We had a great time connecting with community members and talking about how lifechanging MSSP is to our participants.

Attention Nurses in Santa Barbara, CA! We are hiring for a Nurse Care Manager to join our join the dynamic care management team of Partners' Multipurpose Senior Services Program (MSSP).

MSSP provides care management services to low-income, Medi-Cal recipients, who are 65 years or older, frail and at risk of institutionalization. Nurse Care Managers work closely with Social Work Care Managers to conduct home assessments and develop individualized care plans to address the unmet needs of our participants so they can live more independently and safely in their home.

Candidate must have an AA or BS in Nursing and at least one year of experience working with older adults.

On Saturday April 13, the San Fernando Valley Japanese American Community Center hosted a Caregiver Conference Resource Event. This event presented innovated ways of connecting caregiver solutions through technology, which included the implementation of artificial intelligence, service applications, and organizational applications for caregivers to help better serve those in their care.

Additionally, there were various Health Exhibitor Booths present that provided services and information to the community.

Representing Partners in Care Foundation was Nicholas Gonzalez, Community Linkages Specialist. Nicholas provided information on one of our Long Term Services and Supports program, the Multipurpose Senior Services Program (MSSP) and it’s services to the community. He handed out items like brochures, pamphlets, emergency blankets, jar openers, and multi-tool pens.

Each month we recognize two outstanding staff members for their top quality work and team building. Congratulations to Melissa Vitocruz Gopez, MSW, for receiving Employee of the Month for April 2024.

Melissa Vitocruz Gopez has been a part of the Partners in Care Foundation since May 2021. Melissa earned her Bachelor’s Degree in Child & Adolescent Development from CSU Northridge followed by her Master’s in Social Work (MSW) from Simmons University. She joined the Home & Community Based-Alternatives (HCBA) Waiver team with previous experience in Early Head Start teaching, family advocacy, and working with the older adult population struggling with mental illness through individual and group therapies. Her experience working with children, adolescents, families, and older adults prepared her for working with HCBA Waiver participants as it routinely brings multiple demographics together.

Melissa provides care coordination for individuals who are medically fragile to ensure their health and safety needs are being met outside of an institutional setting while also remaining as independent as possible. Melissa shows her commitment to each participant by dedication to timely care coordination, detailed documentation, and individualized monthly contacts. Melissa brings a positive attitude to the team, always willing to offer a helping hand when able.

Additionally, Melissa has been involved in peer onboarding HCBA Waiver new hires, education to grad students and community members interested in HCBA Waiver, as well as participating in a Partners Therapy Piolet Program where she has taken on several individual clients by providing psychotherapy.

Recently Melissa has begun the process of becoming a certified field instructor to assist HCBA Waiver take on GSEWC interns, allowing students to further their education within community care coordination. Melissa’s pursuit to continue professional growth while striving to be a positive influence to those she walks alongside with daily is a testament to her being chosen for employee of the month.

Partners' Multipurpose Senior Services Program (MSSP) serves older adults (65+) living in Los Angeles County, Kern County and Santa Barbara County, and qualified for Medi-Cal. MSSP helps seniors age in place, by delaying or avoiding entirely, unnecessary, and costly nursing home placement through the provision of long-term care services and supports necessary for independent living.

If you are a CBO, hospital, health plan, etc. that works with older adults with complex health conditions, MSSP can help!
