“My mom had a fall last week.”
That’s important information for her doctor. But what happened before, during, and after the fall can be just as important.
At Lifespan, our Care Managers, Caregivers, and Well-Being Activity Coordinators document more than just the incident.
📍 Where and when did it happen?
🚶 What were they doing beforehand?
💙 Were they dizzy, weak, or feeling completely normal?
⏱️ Could they get back up?
📝 Have their mobility, confidence, or activities changed since?
We also document near falls—those moments when someone loses their balance but manages to catch themselves.
Over time, these observations can help identify patterns that might otherwise go unnoticed.
When that information is shared with their physician, it provides a clearer picture of what’s happening at home—not just what’s observed during an appointment.
Because better information helps support better care. 💙
