Can Thermage or Ultherapy help maintain your facelift results?

A facelift can turn back the clock, but what can you do to support your results in the years that follow?

According to Dr. Elbert Cheng, double board-certified facial plastic surgeon with over 25 years of experience, maintaining your surgical results goes beyond the operating room.

Non-surgical energy-based treatments like Thermage and Ultherapy can play a valuable role in long-term facial rejuvenation by supporting collagen production and improving skin firmness and elasticity.

Here's how they work:

• Thermage uses radiofrequency (RF) energy to heat deeper layers of the skin, stimulating collagen remodeling and helping improve skin firmness over time.

• Ultherapy uses focused ultrasound energy to target specific tissue depths, encouraging new collagen production and providing gradual skin tightening and lifting.

While a Deep Plane Facelift and Neck Lift address deeper facial structures and reposition descended tissues, collagen-stimulating treatments focus on supporting skin quality as the natural aging process continues.

The goal isn't to replace surgery. It's to complement your results.

At Cheng Plastic Surgery in the San Francisco Bay Area, Dr. Elbert Cheng emphasizes a comprehensive, long-term approach to facial rejuvenation. From surgical lifting to non-surgical collagen-building treatments, every recommendation is tailored to your anatomy, goals, and stage of recovery.

Wondering how to maintain your facelift results? Schedule a consultation to learn which treatments may complement your surgical outcome once you've fully healed.

What is rhinophyma, and can it be treated?

Rhinophyma is a skin condition that causes the nose to become enlarged, thickened, and irregular in texture. Often associated with advanced rosacea, rhinophyma develops when excess tissue and enlarged oil glands gradually change the shape and appearance of the nose.

While rhinophyma is a benign condition, it can significantly affect a person's appearance, confidence, and, in more severe cases, even their ability to breathe comfortably.

The good news? Rhinophyma can be treated!

This before-and-after transformation showcases the results of rhinophyma treatment performed by Dr. Jacqueline Cheng, a double board-certified facial plastic surgeon with over 25 years of experience.

Unlike traditional rhinoplasty, which reshapes the underlying bone and cartilage, rhinophyma treatment focuses on carefully removing excess thickened tissue and reshaping the outer surface of the nose while preserving its natural anatomy.

The goal isn't to create an entirely different nose. It's to restore a more natural appearance by addressing the tissue changes caused by rhinophyma.

Can rhinophyma come back after treatment? Although treatment can significantly improve the appearance of the nose, rhinophyma may recur over time. Ongoing management of rosacea and appropriate skin care can help support long-term results.

If you've noticed progressive thickening, enlargement, or changes in the texture of your nose, a consultation with an experienced facial plastic surgeon can help determine whether rhinophyma treatment is right for you.

What is the best treatment for under-eye bags and loose under-eye skin?

For patients dealing with both prominent under-eye bags and crepey or lax skin, one treatment may not address everything. That’s why Dr. Elbert Cheng combined Lower Blepharoplasty with Fat Repositioning and CO₂ Laser Resurfacing for this patient.

Now five months post-op, you can see how each part of her treatment addressed a different concern.

Lower Blepharoplasty with Fat Repositioning addresses the underlying fullness that creates under-eye bags. Rather than simply removing fat, the existing fat can be repositioned to soften the transition between the lower eyelid and cheek, helping reduce both puffiness and the appearance of hollowness.

CO₂ Laser Resurfacing works at the skin level, stimulating collagen and improving fine lines, crepey texture, and skin laxity that surgery alone may not fully address.

The result? A brighter, more rested under-eye area without changing the natural shape or character of her eyes.

This is why treating under-eye aging isn't always about choosing between surgery or laser resurfacing. When multiple concerns are contributing to a tired appearance, combining treatments can address both the deeper anatomy and the skin itself.

Five months after surgery, she still looks like herself: just more refreshed.

What procedures can address droopy eyelids, under-eye bags, and facial volume loss at the same time?

At one year post-op, this patient’s results show how combining procedures can address multiple signs of facial and eyelid aging while still maintaining a natural, balanced appearance.

Dr. Elbert Cheng performed a Sub-Brow Lateral Eyelid Lift, Ptosis Repair (Mullerectomy), Lower Blepharoplasty with Fat Contouring, Fat Transfer to the V2 region and cheeks, and CO₂ Laser Resurfacing to the lower eyelids.

Each procedure had a specific role:

• Sub-Brow Lateral Eyelid Lift addressed excess skin and heaviness along the outer upper eyelid.
• Ptosis Repair (Mullerectomy) helped improve eyelid position by addressing a drooping upper eyelid.
• Lower Blepharoplasty with Fat Contouring addressed under-eye bags and helped create a more seamless transition between the lower eyelid and cheek.
• Fat Transfer to the V2 region and cheeks restored volume in areas where facial fullness had diminished.
• CO₂ Laser Resurfacing treated the lower eyelid skin to improve texture and fine lines.

At the one-year mark, swelling has resolved and the tissues have had time to fully settle, making this an important milestone for evaluating long-term surgical results.

Dr. Elbert Cheng is a double board-certified facial plastic surgeon with more than 25 years of experience in facial plastic and reconstructive surgery. His approach focuses on treating the underlying causes of facial aging while preserving the features that make each patient look like themselves.

How long does it take to fully heal after Lower Blepharoplasty? While the biggest changes happen within the first several weeks, the under-eye area can continue to refine for months.

This patient’s healing timeline after Lower Blepharoplasty with CO₂ Laser Resurfacing shows why patience is such an important part of the process:

13 days: Early swelling, redness, and residual bruising are still expected. The under-eye area is already beginning to take shape, but this is very much an early result.

6 weeks: Much of the initial swelling has resolved, and the transition from the lower eyelid into the cheek is becoming more refined. Redness from CO₂ resurfacing may continue to fade.

Several months: The tissues continue to settle, residual swelling gradually improves, and the skin continues to remodel following CO₂ laser treatment.

1 year: The result is fully matured, showing the long-term improvement in the under-eye area after healing and refinement.

Lower Blepharoplasty addresses structural causes of under-eye bags, while CO₂ Laser Resurfacing can complement surgery by improving skin texture and fine lines. Together, they address both the deeper anatomy and the skin itself for a naturally refreshed appearance.

Healing is gradual, and every patient’s timeline is different, but this before and after is a great reminder that the result at two weeks is not the result at six weeks, and the result at six weeks is not the final result.

Can you still get a facelift if you’ve had Sculptra? Yes! And with a Deep Plane Facelift, previous Sculptra may be less of a concern.

In this Microlearning, double board-certified facial plastic surgeon Dr. Jacqueline Cheng explains why. Sculptra stimulates collagen within the tissues where it’s injected, but a Deep Plane Facelift works beneath these more superficial layers, releasing and repositioning the deeper facial structures.

Because the Deep Plane technique approaches the lift at a deeper anatomical level, previous Sculptra treatments generally don’t interfere with the procedure in the same way they might with techniques that rely more heavily on superficial tissue dissection.

That said, your surgeon should always know your complete injectable history (including what you had, where it was injected, and when) before creating your surgical plan.

So, can you have a Deep Plane Facelift after Sculptra? For many patients, yes. A consultation with an experienced facial plastic surgeon can determine the right approach for your anatomy and treatment history.

Why are multiple procedures sometimes combined with a Facelift?

At two months post-op, this patient’s results show how addressing different areas of facial aging together can create a cohesive, natural-looking result.

Dr. Jacqueline Cheng performed a Deep Plane Facelift & Neck Lift, Lower Blepharoplasty with Fat Repositioning, Temporal Brow Lift, Fat Grafting, and CO₂ Laser Resurfacing. While that may sound like a long list, each procedure has a different purpose.

The Deep Plane Facelift & Neck Lift addresses facial and neck skin laxity, while the Lower Blepharoplasty and Temporal Brow Lift rejuvenate the eye area. Fat Grafting restores volume that has diminished with age, and CO₂ Laser Resurfacing addresses the skin itself, including texture, fine lines, and sun damage.

Rather than asking one procedure to address every sign of aging, combining complementary procedures allows different layers and areas of the face to be treated together.

At two months, there is still healing and refinement ahead, but the goal is already clear: a refreshed version of the same person, with her natural features preserved.

Performed by double board-certified facial plastic surgeon Dr. Jacqueline Cheng at Cheng Plastic Surgery in the Bay Area.

How long does it take to fully heal from a Deep Plane Neck Lift?

Deep Plane Neck Lift recovery is gradual, and while the biggest changes happen early, the result continues to subtly evolve for months.

This patient is now 6 months post Deep Plane Neck Lift with Dr. Elbert Cheng, a double board-certified facial plastic surgeon with over 25 years of experience in facial plastic surgery.

As swelling continues to resolve, the tissues soften and settle into their new position. These later changes can be subtle, but they’re an important part of the healing process. The neck and jawline can continue to look more refined and natural as recovery progresses.

By 6 months, patients are much closer to their mature result, although subtle healing can continue beyond this point.

A Deep Plane Neck Lift isn’t just about the initial transformation, it’s about allowing the result time to settle, soften, and mature naturally.
