Welcome to another Gel and Tell!! Where our residents who are on their Ultrasound elective share their interesting finds!

A patient with a history of MI and coronary stent placement presented to the ED with two days of shortness of breath. During bedside cardiac POCUS to assess LV function and cardiac wall motion, we identified significant left ventricular wall motion abnormalities, including markedly reduced septal motion, findings concerning for LAD territory ischemia.

Comparison with a prior echocardiogram revealed a dramatic interval decline in LV function, raising concern for a new cardiac event.

Clinical pearl: Bedside cardiac ultrasound can provide critical, real time insight into cardiac function and help identify concerning changes that warrant urgent further evaluation, particularly in patients with significant coronary artery disease.
