Please don't ask AI to write your rehab program postpartum

There's a reason that clinicians say it depends all the time, people are nuanced.

We need contextualized care, because sometimes what works for most people is the exact opposite of what your body probably needs.

There's so much value to In-Person care when you can access it, and if you're open to an internal examination, nothing can really compare to the information that we can glean from directly contacting tissues

So before you waste time on a program that really doesn't understand your body, talk to a therapist like a real human one please 🩷

Not going to lie, biking has never been my favorite πŸ’€

I've done triathlons where I place in the top ranks for swimming and do really well in the run but am relentlessly passed on the bike

I do not do well with prolonged sitting because I tend to have a pelvic floor that runs tight, my nerves do not enjoy the compression from a bike seat

Managing tension with a pelvic wand can be really helpful, but nerves are also very sensitive to compression, and sitting on a hard narrow seat for an endurance ride. Certainly can compress those nerves

So if you've struggled on the bike like I have, your pelvic floor is likely trying to tell you that it needs a little bit of TLC, and it's a sign that you should probably be talking to a pelvic floor PT

On the flip side, if you feel way more comfortable training in a seated position like biking, then running or standing, your public floor may be trying to tell you that it really needs more support, or you need better pressure management strategies.

I've had women pretty early postpartum jump on a bike and feel like it's one of the only places they can feel really successful in pushing themselves early on

So how do you feel about the bike? Love it or hate it?

And there's so much noise about how we should feel about so many things in motherhood.

The attitudes and advice we absorb from well-meaning friends and family members are not always the most helpful, especially when it comes to intimacy

So let's open up the conversation. Nothing's taboo, there's no TMI, there's no shame, just education based on your body and your brain

Pelvic floor injuries after but are really common

It's important for providers to listen for symptoms, and if they aren't obvious on a quick physical examination, it's probably a good idea to refer them to therapy

The number of times I've had women say that they were all cleared, only to find a symptomatic prolapse on exam is too high to count

If their symptoms are only occurring under load, you probably need to test them there before you tell them that their symptoms aren't real

The presence of a pelvic floor injury increases the risks of postpartum mood disorders. Taking the time to properly screen patients and direct them to treatment is critical

Supporting them with a multidisciplinary approach matters

Realizing that I had a prolapse postpartum it was really hard not to spiral. I felt like big parts of my identity were going to disappear. It was really distressing to feel like I couldn't lift my kids without injuring myself. It was constantly distracting to feel the creeping pressure as my day went on

Moms deserve care that can meet them where they are and help them find a way forward, not dismissal, not half assed assessments, and definitely not snake oil that claims it can fix everything

Waking up those lower abs again should be an EARLY postpartum recovery goals, it's something initiate as early as I can with patients

Those muscles are important to help us lift and carry our babies, stabilize our spine, and retrain the core and pelvic floor to work together to manage pressure

When we don't address imbalances in the core and try to progress our ab exercises too quickly, we can end up with a persistent pooch every time that we try to train

Need help troubleshooting your program? Comment or DM "CORE" to get insights on your core training
