Our CMO Michael Winkleman was at the PHA Leadership Summit in Napa last week, and he came back with more notes than expected.

The policy landscape around physician-owned hospitals is shifting in ways that felt stuck for years. Insurance company CEOs acknowledged in front of Congress that physician-led hospitals increase competition.

CMS is opening the door to more physician involvement in payment models. And the conversations happening in that room, around ownership, AI, joint ventures, and revenue cycle, are exactly the ones ASC leaders should be having right now.

Last week, our team was at the MNASCA 2026 Annual Conference in Minnetonka, MN, one of the most important gatherings for ambulatory surgery center leaders in the region.

The conversations that stood out most were around legislative and regulatory updates, operational compliance, and what it actually takes to build a culture that sustains high performance over time. These are not abstract topics for ASC leaders right now. They are showing up in board meetings, scheduling decisions, and staffing conversations every week.

If you were not able to attend, the takeaways are worth your time. Follow for more on what is shaping the ASC landscape heading into Q4.

Many ASCs complete operational reviews or coding audits, but improvement comes from what happens next. Small adjustments in workflows, staffing alignment, and supply management can create meaningful changes in financial performance.

This case study follows one surgery center's path from financial strain and operational instability to a projected $1.2M EBITDA, and breaks down the operational decisions that made it possible. If your center is navigating similar pressure, this is worth reading.

Outpatient surgical volumes are projected to reach 109.6 million cases by 2033. In most markets, ASC schedules are as full as they have ever been. But a full OR does not automatically mean a healthy margin.

The gap between case volume and actual collections is where surgery centers quietly lose ground. Denial rates at ASCs without structured revenue protection protocols run between 18 and 28 percent. Best-in-class centers operate between 4 and 6 percent. Unresolved implant charge capture, payer-variance gaps, and outdated contracts can erode EBITDA by 15 to 28 percent, even when the schedule looks strong.

Q4 is the right time to find out where your revenue cycle stands before year-end. Read the full breakdown here: https://www.smpsd.com/blog/asc-revenue-leakage-hidden-costs-for-ascs/

As Q3 closes, a pattern is clear among the surgery centers that are performing well this year. They are not necessarily the biggest or the best-resourced. They are the ones that stayed operationally disciplined when margins tightened, kept their physicians aligned when the schedule got complicated, and made decisions based on data rather than habit.

Site neutrality, CMS procedure expansions, and supply chain pressure have reshaped the ASC landscape in 2026. The centers entering Q4 with confidence are the ones that treated those shifts as signals, not noise.

Read how the best ASCs are navigating financial pressure heading into Q4, and follow for more on ASC operations, performance, and what it takes to lead a surgery center in 2026.

Healthcare Quality Week is a moment to recognize the systems, standards, and people who make consistent, safe care possible in surgical settings.

In ambulatory surgery centers, quality is not a checkbox. It is built into every turnover protocol, every credentialing decision, and every conversation between a surgeon and an administrator about what is and is not working. The teams that take quality seriously every day are the ones that earn the trust of their patients and their physicians over time.

Thank you to everyone in the ASC community who holds that standard.

Running an ambulatory surgery center means managing financial performance, clinical oversight, staffing shortages, surgeon relationships, and patient experience, often at the same time.

The most effective administrators do not try to be experts in all of it. They build teams that fill the gaps, ask questions instead of guessing, and stay present enough that their staff actually trusts them. One practical example from the field: standardizing a single supply item, surgical gloves, saved one ASC $36,000 annually. That kind of result comes from structured attention, not luck.

Read the full blog for a breakdown of what today's ASC administrators are navigating and how the best ones are doing it.

Labor Day is a reminder of the dedication behind every industry that keeps the world moving. In ambulatory surgery centers, that dedication looks like an OR team ready at 7 am, an administrator managing three crises before noon, and a billing team closing the quarter without missing a beat.

To every person working in and around surgical care, today and every day, thank you.
