'𝐎𝐧 𝐭𝐡𝐞 𝐑𝐚𝐝𝐚𝐫' | 32nd Edition is live!

PBM reform is gaining momentum, but will greater transparency actually lead to lower pharmacy costs?

In this edition, we cover
• Why bigger PBM rebates aren't translating into lower employer drug spend
• The Big Three PBMs losing ground as employers explore alternatives
• New challenges threatening access to affordable prescription imports
• Drug launch prices reaching new highs
• Growing pressure on employers from hospital and pharmaceutical pricing
• The ongoing debate over prior authorization and medication access

Transparency is only part of the solution. Employers need to look beyond rebates and pricing models to understand total pharmacy spend, clinical value, and the true financial impact on their plans.

Employers are increasingly stepping up as healthcare purchasers, and trusting the old plan model is no longer enough. To control rising costs and hidden fees, leaders must demand full visibility into their spending.

As Paul Pruitt, co-founder and CGO at SHARx, notes: “Transparency should no longer be viewed as a luxury; it should be an expectation. Employers cannot effectively manage any major business expense if they cannot see the underlying data.”

While national policy discussions focus on Most-Favored-Nation drug pricing deals, simply lowering manufacturer costs does not automatically translate to relief at the pharmacy counter.

"All of Pharma seems to have these long-term contractual agreements with the government programs and the commercial markets, and they're all intertwined and interconnected in a very challenging way that is not easy to unwind," explained SHARx Chief Growth Officer Paul Pruitt.

High demand and steep price tags have made GLP-1 medications a massive financial strain for employer health plans. But navigating the economics of these blockbuster drugs involves much more than surface-level sticker shock.

"When it comes to the economics of medications, it is really hard to point your finger at a rational cause," SHARx Co-founder Paul Pruitt noted. "The manufacturing processes aren't necessarily disclosed, so you can't look at that as a contributing factor in a meaningful way."

Political debates over cancer drug prices often miss the hidden specialty pharmacy gatekeepers that dictate whether patients actually receive life-saving treatments.

"The future of oncology will undoubtedly include more sophisticated therapies, more personalized treatment strategies, and continued scientific breakthroughs," writes SHARx Chief Growth Officer, Paul Pruitt. Major employers are cutting 2027 health benefits to curb rising healthcare expenses, leaving workers to assume higher premiums and reduced coverage. However, simply passing costs onto employees fails to fix the root issue, especially when it comes to skyrocketing prescription costs.

As pharmacy expenses continue to outpace medical trends, companies must rethink how they handle high-cost specialty drugs. Pointing toward alternative strategies, Paul Pruitt, Co-founder of SHARx, notes that employers need a new approach, "and maybe that's 'exiting the traditional insurance game.'"
