That’s a wrap on UHCA/UCAL in Sandy, Utah.

Thank you to everyone who stopped by Booth 112 to see Kourtney and Mitch.

Thanks to the Utah Health Care Association and Utah Center for Assisted Living for a well-run show. If we didn’t get a chance to talk, send us a message. We’d love to keep the conversation going.

There’s nothing like getting nursing leaders together in one room.

Faye, John and Mary are heading to NCDONA in Myrtle Beach, and we’re looking forward to conversations with the people who understand the realities of resident care firsthand.

We’ll be talking about how AI-powered analytics, connected vitals and clinical support can help teams recognize meaningful changes sooner and focus their attention where it matters most.

If you’ll be there, come say hello to the TapestryHealth team.

An alert isn’t the intervention.

It’s the beginning of the clinical conversation.

For RPM to actually support better care, the data has to lead somewhere.

What changed?

Is this an isolated variation or part of a larger pattern?

How urgent is it?

Does the resident need immediate attention, or closer observation?

What happens next?

Assessment? A repeat measurement? Provider notification? Medication review? A change to the care plan?

And then one more question: Did it work?

Continued monitoring helps the team see whether the resident stabilized, improved, or needs further escalation.

That closed loop is what makes RPM more than a stream of vital signs.

Technology can surface the signal.

A strong clinical workflow turns that signal into action.

Our latest blog looks at how SNFs can build RPM into care processes designed to identify decline earlier and reduce avoidable escalation.

AI can identify patterns, estimate risk, and help determine which residents may need closer review.
It cannot replace the nurse who knows the resident.

Clinical judgment is still needed to assess the resident, understand the context, and decide what action—if any—is appropriate.

That is why TapestryHealth pairs technology and clinical intelligence with experienced clinical support.

Technology helps organize and surface information. Clinicians remain responsible for interpreting it and making care decisions.

Medical decline doesn’t always announce itself.

Sometimes it looks like a little more fatigue.

A gradual increase in weight.

A resting heart rate that keeps creeping up.

Oxygen saturation trending lower than usual.

On their own, each change may not tell the whole story. But together, they can signal that a resident is beginning to decline.

That’s where Remote Patient Monitoring becomes especially valuable.

By tracking changes over time, RPM can help SNF care teams recognize patterns earlier, assess what’s happening, and determine whether clinical intervention may help keep the resident stable in place.

The goal isn’t more data.

It’s seeing the right changes early enough to do something about them.

Our latest blog explores how RPM can help SNFs identify subtle signs of decline and create more opportunity for proactive intervention.

Not every skilled nursing facility needs the same AI solution.

A facility may need help prioritizing residents for review. Another may need to bring information from several workflows into one clearer view. Another may need to reduce the number of separate systems staff must navigate.

The right starting point is not the technology.

It is the problem affecting staff and resident care.

TapestryHealth combines clinical expertise, data analytics, and technology-enabled support to help facilities turn information into a more useful and actionable clinical picture.

Is the reading concerning?

There’s another question that may matter just as much:

Is it different from this resident’s baseline?

A single blood pressure or oxygen reading doesn’t always tell you enough.

The more meaningful signal may be the direction things are moving.

Is oxygen saturation gradually decreasing?

Is resting heart rate rising?

Is weight increasing?

Are new symptoms appearing alongside those changes?

That context matters.

Effective Remote Patient Monitoring isn’t about reacting to every number. It’s about helping clinical teams recognize meaningful changes from baseline, understand them in the context of the resident, and focus attention where it may be needed most.

That’s the difference between simply collecting vitals and using them to support proactive care.

We take a closer look at this in our latest blog.

Sepsis is the body’s extreme response to an infection and is a life-threatening medical emergency.

For older adults, recognizing a concerning change and escalating it for clinical review can be especially important. Awareness, timely communication, and access to the resident’s clinical context all support informed decision-making.

This Sepsis Awareness Month, help keep recognition and rapid response top of mind.

Falls are not always isolated events. A change in balance, mobility, medication, cognition, or overall condition may signal that a resident needs closer attention.

Falls prevention starts with awareness, consistent observation, and communication across the care team.

When teams can see meaningful changes sooner, they are better positioned to review risk and take appropriate action.

Washington + Idaho, you showed up.

Cory Huot spent last week at the WHCA/IHCA Joint Fall Conference, and the conversations were nonstop.

From reconnecting with familiar faces to meeting new operators and care teams, there was no shortage of honest discussion about what’s happening in skilled nursing right now, where the pressure points are, and where better clinical support can make a real difference.

That’s what makes these conferences worth it.

Good people. Real conversations. New connections. And plenty to keep talking about long after everyone heads home.

Thank you to everyone who took the time to connect with Cory and TapestryHealth. We loved being part of it.

Washington and Idaho, we’ll see you again soon.
