“My vitamin D is low” tells me something.

It doesn’t tell me everything.

I want to know the actual number. I want to know whether you’re having symptoms, what your health history looks like, whether you’re already taking vitamin D and how much you’re taking.

Because there’s a difference between casually adding another supplement to your morning routine and actually treating a documented deficiency appropriately.

Your lab result gives us information. What we do with that information should be individualized to you.

“Just take some vitamin D” isn’t much of a treatment plan.

Yes, we need to talk about this part too.

Some medications used for male pattern hair loss work by reducing the effects of DHT, and s*xual side effects can occur, including changes in libido, erections or ej*******on.

That does not mean they happen to everyone. And it doesn’t mean you should automatically be afraid of treatment.

It means you deserve the whole conversation.

I want you to understand what a medication may help, what its potential risks are, what to watch for and what we’ll do if something changes.

Hair matters. Your s*x life matters. Your comfort with the treatment matters.

Informed decisions require talking about all of it.

If I mention metformin and your immediate response is, “But I’m not diabetic,” you’re definitely not the first person to say it.

Metformin is well known as a diabetes medication, but that isn’t the only context in which it may be prescribed.

For some patients struggling with weight management, insulin resistance and metabolic health may be part of the bigger picture. That’s why I’m interested in more than the number on your scale. I want to understand what may be making weight management harder for your body.

And no, that doesn’t mean metformin is automatically right for you.

The point is never to prescribe the medication that happens to be popular. It’s to understand what’s happening in your body and choose treatment accordingly.

I promise, talking about erectile dysfunction with me is probably going to be much less awkward than you’ve built it up to be in your head.

And yes, we can absolutely talk about medication. I want it to work for you.

But ED can sometimes tell us something about what’s happening elsewhere in the body. Cardiovascular health, blood sugar and metabolic health, hormones, medications and other factors can all play a role.

So I’m going to ask a few questions before we skip straight to the prescription.

Not because I want to make the conversation uncomfortable. Because I care about your health beyond whether the medication works Friday night. 😉

“I’m barely hungry” can sound like proof that your weight-loss medication is doing exactly what it’s supposed to do.

But I don’t want the goal to be eating as little as humanly possible.

Your body still needs protein and other nutrients. It still needs hydration, sleep and movement. And if you’re consistently struggling to eat enough, getting full after a few bites, feeling weak or nauseated, or skipping meals because food simply doesn’t appeal to you, I want to know.

Appetite suppression can be helpful. Being unable to adequately nourish yourself isn’t the finish line.

Weight loss should support your health, not compete with it.

If your dad has a full head of hair while yours seems to be making an early exit, you might be wondering how genetics managed to pull that one off. 😏

Male pattern hair loss is genetic, but inheritance is a lot more complicated than comparing your hairline to your dad’s or your maternal grandfather’s.

Multiple genes can play a role, and they can come from both sides of your family. What also matters is how sensitive your hair follicles are to DHT, a hormone made from testosterone. In genetically susceptible follicles, DHT can gradually cause the follicles to shrink, producing finer, shorter hairs over time.

So yes, Dad may have won the hair lottery while you did not. Genetics can be rude like that.

If you’re noticing a receding hairline, thinning at the crown, or changes that seem to be progressing, it may be worth talking about sooner rather than waiting to see how much hair you lose.

One of the first things I want you to know about a GLP-1 plateau is that it doesn’t automatically mean we need to increase your dose.

And it definitely doesn’t mean you failed.

If the scale has slowed down or the food noise that had gotten so quiet suddenly starts getting louder again, I want to know what else is happening.

Has your actual physical hunger returned, or are you thinking about food more? Did this happen gradually or suddenly? Has your sleep changed? Stress? Routine? Other medications? How long have you been on your current dose?

Those details matter because bodies adapt, and weight loss rarely happens in one perfectly straight line.

I don’t want to chase every plateau with more medication. I want to understand what the plateau is telling us first.

Your dose is part of the conversation. It shouldn’t be the entire conversation.

If I had a dollar for every time I heard, “I swear I’m not eating more, but I’m gaining weight,” I’d have quite a few dollars.

And one of the first things I want you to know is that I believe you.

When your weight starts changing, it’s easy to assume you must be eating too much or doing something wrong. But weight regulation is more complicated than calories alone. Sleep, stress, medications, insulin resistance, hormonal changes and metabolic health can all influence what’s happening.

So when someone tells me, “Nothing has changed, but my body has,” I don’t hear an excuse. I hear information.

That’s when we start asking better questions. What changed with your sleep? Your medications? Your cycle? Your stress? Your hunger? Your energy? Your labs?

You shouldn’t have to keep cutting food and working harder without understanding what you’re actually trying to fix.

Sometimes the answer isn’t eating less. Sometimes we need to figure out what changed.

The “baldness gene comes from your mom’s side” explanation is one of those things that gets repeated so often it starts to sound like settled science.

It’s just not that simple.

Male pattern hair loss is influenced by genetics from both sides of your family, along with how sensitive your hair follicles are to hormones like DHT. Over time, susceptible follicles can begin to shrink, producing hairs that are finer, shorter and eventually much less noticeable.

And that matters because noticing a changing hairline or thinning crown doesn’t mean you simply have to shrug and say, “Well, Grandpa was bald too.”

Understanding what’s actually happening is a much better place to start. If you’re noticing changes in your hair and wondering whether they’re worth addressing, that’s a completely reasonable conversation to have. We can look at what you’re seeing, your history and what options may make sense for you.

Vitamin D is one of those things you may not think about until you find out yours is low.

Maybe you’re tired more often than you think you should be. Your muscles feel weaker or achier. Your mood has been a little off. You’re getting sick more often. Or maybe you don’t have an obvious symptom at all.

That’s the tricky part. Low vitamin D doesn’t always announce itself in a way that makes you think, “I should have my vitamin D checked.”

I’d much rather know what your level actually is than have you randomly start taking a supplement because you saw someone online say everyone needs one.

A simple blood test gives us somewhere to start. From there, we can talk about your level, your health, whether supplementation makes sense, and when it should be checked again.

Sometimes feeling “a little off” is worth looking into, even when you can’t quite put your finger on why.
