Denied insurance claim? Start by checking the reason code first 🧾

Most denials fall into a few common buckets:

• missing prior authorization
• services billed out of network
• claim submitted with missing or incorrect info
• service not covered under the plan
• filing deadline missed

As a former utilization review nurse, I know those denial codes usually tell the real story before anyone on the phone does.

If you want help understanding what your denial actually means, schedule a free consultation or ask about the Book now link.

Still chasing answers on a specialist bill that should have been explained weeks ago? 👀

Before: repeated calls, unclear charges, and no simple breakdown of what was actually owed.

After: corrected charges, a clean billing review, and a manageable next step plan the client could actually use.

This anonymized case shows how a Patient Advocate can step in to review the bill, compare it against coverage, identify errors or items needing clarification, and create an organized plan for follow-up.

Because when the bill is confusing, the next step should not be.

Confusing medical bill? Don’t call in a panic just yet 📄

Before you dispute it, gather these 4 documents first:

1. The provider bill
This shows the actual charges, dates, and line items. A Patient Advocate uses this to flag duplicate charges, coding issues, and surprise add-ons.

2. Your EOB
Your Explanation of Benefits shows what insurance processed, paid, or denied. A Patient Advocate compares this to the provider bill to catch mismatches fast.

3. Your insurance ID details
Group number, member ID, and plan info matter more than most people realize. A Patient Advocate uses this to confirm the claim was billed to the correct plan with the right policy details.

4. Visit notes or discharge paperwork
These help verify what care you actually received. A Patient Advocate can use them to spot charges for services that don’t match the visit.

Think of it like billing detective work—less drama, more documents.

Save this post for later, and if you want a Patient Advocate to help review a confusing bill, use the Book now link for a free consultation.

Before you pay that hospital or specialist bill, check these 3 line items first. 👀

Billing errors can happen when life is busy, and those small mistakes can add up fast.

Simple bill checklist:
✅ Duplicate charges
✅ Out-of-network surprises
✅ Incorrect insurance adjustments

If the same service appears twice, if a provider was treated as out-of-network when you were told otherwise, or if your insurance payment or adjustment does not look right, pause before paying.

A Patient Advocate can help review what looks off and explain the next step clearly. 💡

If any item on this checklist raises a red flag, book your Free Consultation today.

Empower your GLP-1 appeal

07/28/2026

Need your GLP‑1 appeal to look as precise as that syringe in the photo? 💉

When a reviewer opens an appeal packet, they want clear, clinical information — not a grocery-list of meds. Start by organizing: a one-sentence clinical summary from your prescriber, a medication timeline, and dated progress notes that match the dose and response shown in the record.

Why those three things matter:
1️⃣ Clinical summary — gives the reviewer the “why” in one breath
2️⃣ Medication timeline — shows previous therapies and why GLP‑1 is next
3️⃣ Provider notes with dates and measurements — proves medical necessity

Think of the syringe and vial image next to your appeal: reviewers expect the technical details to be just as tidy and exact. A neat, clinician-friendly packet reduces back-and-forth and speeds decisions — and that’s the difference between a yes in weeks vs. months of hold music.

If you want a ready-made structure and wording that reviewers respond to, our GLP‑1 Medical Necessity Appeal Letter toolkit walks you through each section step-by-step. Grab the toolkit or DM us for a free checklist to start your appeal the right way.

Ever wonder what I’m actually looking at when I review a medical bill? 👩🏽‍⚕️

In this quick founder video, I’m at my desk showing how I match an EOB against the provider bill to see whether the remaining balance points to a coding issue, an insurance processing problem, or true patient responsibility.

This is the same behind-the-scenes method I use to help clients stop guessing and start asking the right questions.

Because not every balance means you owe it — sometimes it means something got missed along the way.

Press play, and if your bill has you going in circles, book a free consultation.
