Chronic Certification Center provides in office visits, remote visits, online instant medical certification cards and assistance and education to individuals in the process of applying to the Michigan Medical Ma*****na Program (MMMP). Our licensed physicians, in compliance with the Michigan Medical Ma*****na Act, review patient conditions and consult with the patients to determine if they are cand

idates for medical ma*****na use. If deemed appropriate, the doctor will provide a signed Physician Certification Form or electronic form to the client, thereby permitting the client to apply for the Michigan Medical Ma*****na Card with the Michigan Department of licensing and regulatory affairs.
