What happens when an inaccurate diagnosis follows someone beyond the treatment room?

It can affect their employment, child custody, access to medical care, and even how the justice system treats them.

The DSM-5-TR no longer recognizes polysubstance use disorder as a standalone diagnosis, yet the term persists in treatment settings.

When we use a broad diagnostic label instead of examining which substances a person uses and why, we risk overlooking trauma, co-occurring mental health conditions, and other factors that influence treatment outcomes.

In my latest article for Vital Voyage, I examine the consequences of misusing this diagnosis and offer practical recommendations for more accurate assessments, less stigma, and better individualized care.

If you work in substance use disorder treatment, this is an important conversation worth having.

ON MORAL INJURY & SUD 👇

Recovery often requires people to confront painful truths about the harm they caused, the relationships they damaged, and the values they violated.

Yet that is only part of the story.

Many must also come to terms with what was done to them, the betrayals, losses, neglect, violence, and impossible circumstances that shaped their journey.

Recovery is not simply a process of stopping substance use; it is often a struggle to reconcile both what happened to us and what we did in response.

Too many grandparents are being forced into an impossible choice: protect their grandchildren or preserve the relationship with their own child.

When substance use disorder enters a family, it doesn’t just impact the person using. It reshapes the entire family system. Grandparents often must step in while they watch their grandchildren carry trauma and their child live with fear, grief, and impossible decisions.

In this deeply personal article, I share what it was like to navigate my daughter’s opioid use disorder, family court, child safety concerns, and the emotional toll of becoming a caregiver again.
