CMS is Transforming Kidney Care in America

The End Stage Renal Disease (ESRD) Network Program is revolutionizing how we deliver kidney care through four key priorities:
• Empower Patient & Family Choice - Putting control back in patients' hands with comprehensive information about treatment options and supporting shared decision making
• Improve Care Coordination - Breaking down silos between dialysis facilities, transplant centers, and healthcare providers for seamless, integrated care
• Promote Whole Health - Addressing physical, mental, and social health needs —not just treating kidney disease
• Enhance Data Interoperability - Ensuring seamless data exchange to reduce duplicate testing and support better decision-making

Together, we're working toward optimal kidney health for all patients across America!

Keeping Dialysis Patients Safe —Every Day and During Disasters

Did you know dialysis patients can't miss treatments, even during emergencies? After Hurricane Katrina revealed critical gaps in care, CMS created a powerful partnership to protect America's kidney patients. End Stage Renal Disease (ESRD) Networks & ESRD Kidney Analytics and Responsive Emergency Support (KHARES) work together as a national-to-local system:

• 18 ESRD Networks respond locally with state-specific disaster plans and real-time facility monitoring
• KHARES coordinates nationally with 24/7 helplines, emergency summits, and data analytics.
• Together, they are driving improvements in patient choice (home dialysis & transplant), care coordination (reducing hospitalizations), whole health support, and data interoperability.

This partnership supports the Advancing American Kidney Health Initiative goals, including increasing home dialysis/transplant rates to 30% by 2025 and reducing ESRD development by 25% by 2030. High-quality care every day. Life-saving support when disaster strikes.
