IntouchCM has been collaborating with Everlasting since 2024, when they were a vendor at Need For Speed Marketing Phoenix Senior Living Expo, and we are so glad to now let others know about their amazing services. Their communities offer a lot of on-site services as well as mobile P*P, Wound Care, Therapy, Podiatry, and much more.
The referral process is quite simple – you can call Alicia directly at 602.830.2161 (it rings to her desk and cell and has confidential voicemail), or you can email her at [email protected]. Whatever works for you!
From there, she takes some basic info (name, dob, dx, what types of care the resident will need – whether it is ADL’s, support, cues and reminders, behavioral, etc. ). They also have an SCU (Special Care Unit). While their entire 124-bed facility is secure with all entry/exit points controlled by employee badge swipe, the SCU is even further secured and residents there require higher levels of care or are potentially greater elopement risks or have more behaviors.
Because they are predominantly Brain Injury, with TBI, CVA, anoxic injuries, etc. They have a spina bifida residents who does not have cognitive deficits, but requires assistance with ADLs, and residents who have a long history of poly-substance abuse and have cognitive issues as a result. Alicia is always happy to do an assessment (at the hospital, rehab, post-acute, client’s home, etc) to further determine candidacy for placement.
